中文摘要 | 背景與目的:探討1996-97年金門縣金城鎮第2型糖尿病患者糖尿病視網膜病變發生率及危險因子。方法:以1992-94年透過陽明十字軍針對金城鎮所篩檢出360名30歲以上第2型糖尿病患者為基礎,於1996-97年由眼科專科醫師利用間接眼底鏡進行糖尿病視網膜病變檢查,共有137名完成兩次檢查,以此評估視網膜病變與相關生理學變項的關係。結果:在136名受訪者中,117名第2型糖尿病患者於1996年無合併視網膜病變,經過一年的追蹤有14名新發個案,年發生率為12.0%。在18名背景期或前增殖期視網膜病變患者中,有6名發生惡化,轉移率為33.3%。增殖期視網膜病變的轉移率則為11.1%。t檢定發現糖尿病視網膜病變與糖尿病得病期間與尿素氮有關,進一步由邏輯式迴歸分析可知,糖尿病得病期間(勝算比:1.2,95%信賴區間:1.0-1.3)為影響視網膜病發生最顯著之危險因子。結論:本研究發現金城鎮第2型糖尿病患者其視網膜病變發生率及轉移率偏高,糖尿病得病期間為影響視網膜病發生最顯著之危險因子。 |
英文摘要 | Purpose: To study the incidence and risk factors of retinopathy among type 2 diabetes in 1996-97 in Kin-Chen, Kinmen.Methods: A follow-up study on retinopathy among 360 type 2 diabetes, screened out by the Yang-Ming Crusade from inhabitants aged 30 and over in Kin-Chen in 1992-94, was carried out in 1996-97 by the same ophthalmologist using indirect ophthalmoscopy to examine fundus after dilating pupils for the diagnosis of diabetic retinopathy. A total of 136 subjects were screened two rounds during 1996-97. The relationship between retinopathy and medical/biochemical parameters were assessed. Results: A total of 117 out of 136 subjects had no diabetic retinopathy in 1996. Fourteen subjects developed diabetic retinopathy one year later and the annual incidence was 12.0%. Eighteen subjects had background or preproliferative retinopathy, evidence of deterioration developed in six subjects. The incidence of progression was 33.3%, including the incidence of progression to proliferative of 11.1 %. The T-test disclosed that the development of retinopathy was associated with duration of diabetes and blood urea nitrogen. Multiple logistic regression revealed that duration of diabetes (OR=1.2, 95%CI: 1.0-1.3) was the most significant risk factor related to the development of diabetic retinopathy.Conclusions: The relative higher annual incidence and progression of diabetic retinopathy in Kin-Chen was found in this study. Duration of diabetes was the most significant risk factor correlated with the incidence to the development of diabetic retinopathy. |
