中文摘要 | 膽固醇 (cholesterol) 的來源,一部分由腸道吸收膽固醇,一部分由肝臟內生性合成膽固醇。紅麴作用機轉與 statin 類藥品同作用在 HMG-CoA reductase,但紅麴是經天然發酵而來,除具有降血脂作用外,還含有 GABA 具 血壓之功效,紅麴是 statin 類藥品的替代藥物。益生菌降低血脂之作用經由與膽酸結合而增加膽酸由糞便排除和與膽固醇結合而增加飲食中的膽固醇由糞便排除,與新一類之降血脂藥品 ezetimibe 選擇性抑制食物及膽汁排出之膽固醇在小腸絨毛之吸收,益生菌與 ezetimibe 作用類似皆是經由抑制腸道吸收膽固醇。 同時使用抑制肝臟合成膽固醇的 statin 類藥品與經由胃腸道作用降低膽固醇的 ezetimibe,降血脂作用具有協同效果,但有些藥品副作用,如能以天然健康食品,作為預防及早期治療的替代方案,以預防心血管疾病的發生,不失為可行之法,若同時使用抑制肝臟合成膽固醇的紅麴與經由胃腸道作用降低膽固醇的乳酸菌桿菌兩者合併使用,兩者合併使用抑制膽固醇能增加降低膽固醇的效果,是否具有協同作用值得進一步研究。 |
英文摘要 | Cholesterol absorbed from intestine is a mixture of exogenous and endogenous cholesterol. In fact, dietary cholesterol represents only about one-third of the total intestinal cholesterol pool, while endogenous sources, the bile and to some extent intestinal mucosal cholesterol account for the remaining two-thirds. HMG-CoA reductase inhibitor, like red yeast rice is a fermented rice product, has gained popularity because of its properties as a natural statin. Red yeast rice can decrease cholesterol and triglyceride levels. GABA (γ-aminobutyric acid), a hypotensive agent, is a secondary metabolite produced from red yeast rice which make it an alternative therapeutic option to be used in place of blood lipid-lowering statin drugs. The possible hypocholesterolemic effects of lactic acid bacteria may be mediated by: enhanced bile acid excretion into feces by bile acid deconjugation, enhanced dietary cholesterol excretion into feces by direct cholesterol assimilation. Ezetimibe is a new lipid-lowering drug, a cholesterol absorption inhibitor. It selectively inhibits absorption of cholesterol excreted from foods and bile by the villi of small intestines. The cholesterol-lowering effect of probiotics is similar to ezetimibe. Statin/ezetimibe combination therapy, this dual inhibition results in a significantly greater LDL-C reduction and subsequent achievement of treatment goals, might have serious adverse effects. Using natural products to prevent cardiovascular diseasr is an alternative method. The combination of red yeast rice (which can inhibit the cholesterol synthesis in the liver) and probiotics (which lowers the cholesterol through acting in the gastrointestinal tract) could enhance the choleaterol-lowing effect, whether if having synergistic effect, need further trial to evaluate it. It is worth studing this combination thrapy in hyperlipidemia patients. |
