中文摘要 | 縮短民眾就醫等候時間,為提升醫院工作效率及服務品質的指標之一。本院由病患抱怨之經驗得知,等候看病時間滿意度低與等候病歷送達診間之時間有關,尤其是現場掛號與掛雙診的病歷。本小組收集資料發現現場掛號及掛雙診病歷在30分鐘内送達診間之比率分別為59.9%與52.3%。分析其原因為:診間護理人員不知病患掛雙診、病歷室與診間距離遠、舊院區輸送人員為兼任無法專心輸送、病歷輸送車小且重推送耗時。本專案藉由增加兩名病歷輸送人員、病歷輸送人員佩戴無線對講機、舊院區門診增設收發轉送站、掛雙診之病歷夾内放置粉紅色雙診牌提示、診間電腦增設病患掛雙診之提示系統、增設電動車輸送病歷及舉辦工作人員在職教育等方法改善作業後,經評值發現現場掛號及掛雙診之病歷,在30分鐘内送達診間率提高至84.7%與86.2%,達到本專案設定之目標。 |
英文摘要 | To sharten the waiting time of patients in outpatient departments (OPDs) can promote efficiency of work and quality of service in teaching hospitals. By previous records of patients' complaints, the lower level of satisfaction was usually resulted from the prolonged period of patients' medical charts arrival, especially for those patients who made the registration on site or those who wanted to visit two separate OPDs. Our research team collected the corresponding data and found that the percentages of registration on site and double registrations with medical charts arrivals within 30 minutes were only 59.9% and 52.3%, respectively. After further analysis, the most common causes of such conditions were as the followings: (1) the OPD nurses were not informed about patient's double registrations; (2) long distance between medical chart rooms and OPD rooms; (3) the persons in old hospital building area were part-time and not specifically in charge of medical chart transfer; (4) the transfer vehicle of medical charts were too small in capacity and too heavy to move. This proposal was aimed to improve the problems mentioned above by the following solutions: (1) two addition personnel for transferring medical charts between OPDs and medical chart rooms; (2) wireless phones equipped for the transfer personnel; (3) transfer stop added in the OPD area in the old hospital building; (4) a pinky bookmark paper inserted in the medical chart for indicating the double registrations; (5) automatic alarming system on the OPD computer screen for those patients with double registrations; (6) automatic vehicles for facilitating medical charts transfer and (7) continuous education programs for those transfer personnel. The results demonstrated that the percentages of registration on site and double registrations with medical charts' arrivals within 30 minutes were significantly improved up to 84.7% and 86.2%, respectively, which achieved the target levels we set. |
