中文摘要 | 高血鉀是慢性腎衰竭病患常見的電解質問題。我們發現在我們門診及透析患者中有許多病患在服用中藥。這些中藥有各種型式,包括藥粉、藥丸及草藥等,但不見得都是經由中醫師開立的。因為懷疑中藥可能會造成慢性腎衰竭病患的高血鉀症,我們選擇常使用的五種科學中藥:濟生腎氣丸,導水茯苓湯,越婢加朮湯,右歸丸,左歸丸來測定其中的鉀離子含量。測定方法是利用感應偶合電漿原子發射光譜儀(Inductively Coupled Plasma atomic emmision spectrometry)來分析。分析結果發現,在一般治療劑量下(每天3次,一次5gm)平均鉀離子的攝取量分別為100.2 mg , 119.5 mg 、123.4 mg 、84.6 mg及46.9 mg (相當於鉀含量2.5 mmol,2.9 mmol,3.0 mmol,2.1 mmol,1.2 mmol)也就是說,以一個50 kg的成人為例,他多攝取了5.0%、5.8%、6.0%、4.2%及2.4%的鉀離子。結論:中藥的鉀離子含量,沒有之前認為的高。 |
英文摘要 | Hyperkalemia is a major electrolyte problem often encountered in chronic renal failure patients. In a review of the drug, food, and dietary supplement intake of patients in our out-patient and hemodialysis department we found that many patients had taken Chinese traditional herbal drugs (CTHDs) in various forms (powder, pill, crude herb drug) with or without a prescription/mm a well-trained Chinese traditional doctor. Due to our suspicion that CTHDs may induce hyperkalemia in chronic renal failure patients, we determined the potassium content of five of the most frequently used CTHDs: Ji Sheng Shen Qi Wan, Dao Shui Fu Ling Tang, Yei Bi Jia Zhu Tang, Yo Gui Wan, and Jo Gui Wan. An Inductively Coupled Plasma Atomic Emision Spectrometry was used to check the total potassium in the five CTHDs At the therapeutic dose of these drugs (5 gm powder t.i.d.), the average daily potassium intake was determined to he 100.2 mg. 119.5 mg, 123.4 mg, 84.6 mg. And 46.9 mg, respectively (2.5 mmol, 2.9 mmol, 3.0 mmol, 2.1 mmol, and 1.2 mmol, respectively), which represents an additional intake of 5.0%, 5.8%, 6.0%, 4.2%, 2.4%, respectively, in a 50 kg man. We conclude that the potassium content of CTHDs is not as high as previously thought. |
