中文摘要 | 國家建設包括實施全民健康保險,社區導向之基層照護將成為醫療照護的主幹。在目前十大死亡原因中之腦血管疾病及心血管疾病都與高血壓有關下,如何早期預防並有效控制將有助於減少這兩類主要疾病發生。本文目的在了解都會型集合式國民住宅居民對高血壓的察覺及控制縱狀況。 本研究計劃以臺北市某一國宅年滿20歲以上之成年人為研究對象,以隨機抽樣方式抽得800位為研究對象進行問卷調查及測量血壓。扣除行動不便及不願意參與本研究者後,以共有769位居民完成問卷並測量血壓。 結果發現有138位(17.9%)曾被診斷為高血壓,男性較女性高(22.5%比14.5%,p<0.01),且隨著年齡增加而上升(20至39歲1.7%;40至49歲8.4%;50至64歲26.5%;65以上40.3%,p<0.001),又舉血壓病史之居民中,血壓值控制在<140/90毫米汞柱只有25.4%。在無高血壓病史之631位居民中,174位(27.6%)血壓值>140/90毫米汞柱,女性較男性多,且也隨著年齡的增加而升高,從20至39歲的13%遞增至65歲以上的52.1%。高達93.8%的受訪者認為某一國宅需要成立「社區醫療中心」,以便提供健康諮詢和量血壓等簡易醫療服務。 目前臺北市都會型社會,雖然知識水準已很高,但是高血壓的察覺率及血壓控制良好的比率卻不成正比。 另一方面因為者市內可利用的空間減少,集合式住宅是必然的走向。因此如何利用國宅充分之土地、資源及配合人力,規劃屬國宅的「社區醫療中心」,以便提供居民便利性的健康照護,對高血壓等慢性疾病的防治應有一定的助益。 |
英文摘要 | National development encompasses the implementation of a national health insurance, a community oriented primary health care will be a mainstream in the modern medical care. It was well known that early prevention and effective control of hypertension will decrease the top ten cerebrovascular and cardiovascular mortality. With that, this study was conducted in search of better understanding the awareness and control of hypertension among the residents in urban collective housing projects. This study focuses on adults over 20 residing at the Cheng Kong Collective Housing in Taipei City, a total of 800 residents have been randomly selected to conduct the questionnaire and blood pressure (BP) measurement. After excluding the disable and those refuse to participate, a total of 769 questionnaires with BP records have been derived. The findings showed that 138 (17.9%) had been diagnosed as hypertensive ; the percentage is higher among men than women (22.5% vs 14.5% , p<0.01) and increased with age (1.7% among 20-39 year old ; 8.4% among 40-49 years old ; 26.5% among 50-64 years old and 40.3% for those above 65 year old, p <0.001). Among the residents with hypertension, only 25.4% with the BP controlled <140/90 mmHg. Among 631 residents without past history of hypertension, 174 (27.6%) BP > 140/90 mmHg, the percentage is higher in women and increased with age (13% among 20-39 year old to 52.1% for those above 65 year old). Up to 93.8% of the respondents expect the 「Community-Based Health center」for health consultation and blood pressure measurement. In Taipei, even under the high educational status, the awareness and control of hypertension are unsatisfactory. On the other hand, with the steadily reduction of available urban space, more collective housing projects are inevitable. How best to fully utilize the space; resources and manpower at collective housing to organize a 「Community-Based Health Center」to provide easy accessable health care will help preventing hypertension and other chronic diseases. |
