中文摘要 | 懷孕是一種神秘且複雜的過程,然而內分泌失調、機械性傷害、感染 都可能造成懷孕失敗。近年來研究發現:母體之第二型輔助性T細胞 (T Helper 2 cell ; TH2 ) 和第一型輔助性T細胞 (THelper 1 cell ; TH1 ) 所釋出之細胞激素 (cytokine ) 會影響懷孕之成敗 。所以可依此理論來解釋不明原因之流產 ( 佔 所有流產之60-70% )。國外有些研究室發現:動物體內屬於TH2之cytokine 濃度較高時【如白血球間素-4(Interleukin-4 ; IL-4 ), Interleukin-10 (IL-10)】 對懷孕有正面的影響。而屬於TH1之cytokine濃度較高時【如干擾素 ( Interferon-γ; IFN-γ),腫瘤壞死因子 ( Tumor necrosis factor-β; TNF-β), Interleukin-2 ( IL-2 )】則易發生流產。然而,有學者指出:母子間組織缺乏相 當數量的T 細胞,無法誘導TH2 免疫反應,同時也發現一些不屬於 TH2 cytokine,如:TNF-α,IFN-γ的存在。因此未來還須更詳盡的研究資料來佐 證不明原因的流產與免疫系統之關聯。 K疫系統之關聯。 |
英文摘要 | Pregnacy is a mysterious and complicated natural process in which factors such as hormonal imbalance, mechanical injuries and infections can lead to abortion. Recent studies showed that the release of cytokine from the matermal T helper 2 cells (TH2) and T helper 1 cells (TH1) could affect the outcome of pregnancy. Indeed this theory could help explain 60-70% of all unexplained abortions. Studies abroad with animal models reveal a direct relationship of increased cytokine production from TH2 (e.g. interleukin-4 [IL-4] and interleukin-10 [IL-10]) with a successful pregnancy where as increased cytokine dervied from TH1 (e.g. interferon-([IFN- γ ],tumor necrosis factor- β [TNF- β ]and interleukin-2[IL-2] has the opposite effect. Some authors also discovered that T cell levels are nil to absent at the materno-fetal tissue interface, together with findings of some other cytokines that are not derived from TH2, such as TNF- α,IFN- γ Thus, further studies are needed to elucidate the causal relationship of immune system and unexplained abortions.
