中文摘要 | 肝癌是肝臟最常發生的惡性腫瘤,根據衛生署91年的調查報告,肝癌已列為台灣地區主要癌症死亡原因的第一位。對於肝癌的治療方法,往往需要依賴腫瘤在病人的成長方式及臨床狀況評估後,才能選定合適的治療方式。經皮注射法是目前肝癌治療較為廣泛使用的一種方法,尤其是針對小型、多發、血管供應不豐富之病灶效果最好。本研究嘗試使用兔VX2肝臟腫瘤動物模式,比較各種較常使用在肝癌化學栓塞治療的藥劑,經由超音波導引後直接經皮在腫瘤內注射後的療效。本研究將適當濃度的cisplatin、epirubicin、fluorouracil及mitomycin等四種化學藥劑經皮腫瘤內注射,與經皮腫瘤內注射酒精治療,另選未經治療的作為對照組,比較酒精與此四種化學藥物治療後的效果,期望能找出較理想之經皮腫瘤內注射藥物。經研究結果證實,肝臟腫瘤使用經皮腫瘤內注射酒精,確實可抑制腫瘤成長速率、延長實驗動物存活時間;而fluorouracil的治療效果則優於酒精;mitomycin亦有相同於酒精的治療效果。 |
英文摘要 | Hepatocelullar carcinoma (HCC) is one of the most frequent malignant tumors. According to the statistic data of Department of Health in Taiwan, malignant tumor is the leading causes of death in 2002, and HCC is the leading of all cancer death. The treatment of HCC should depend on the growth pattern of tumor, and the best therapeutic regimen is made after considerate clinical evaluations. Percutaneous injection is one of the most popular methods in treating HCC; especially good results are reported to be in the small, multiple, and less vascular tumors. This study is attempting to apply the VX2 hepatic tumor model of Rabbit for comparison of the therapeutic effect of different chemotherapeutic agents commonly used in chemoembolization of HCC with the aid of ultrasound guided intratumoral injection. An adequate concentration of cisplatin, epirubicin, fluorouracil, and mitomycin was injected directly to the tumor percutaneously. Besides, we also did percutaneous intratumoral injection of ethanol, and selected nontreated tumor as a control group. We compared the therapeutic effect of alcohol injection with these four chemotherapeutic agents, in order to find the ideal percutaneous intratumoral therapeutic drugs. After this study, we concluded that intratumoral injection of ethanol to the hepatic tumor definitely could suppress tumor growth and prolong experimental animal survival time. The therapeutic effect of fluorouracil is better than alcohol, and mitomycin had the same therapeutic effect as alcohol. |
