中文摘要 | 本研究目的在探討不同肥胖程度男性個案,其脈搏傳導速度(pulse wave velocity, PWV)與心臟血管危險因子之相關性。本研究為橫斷式研究,以某職場員工為研究對象,研究工具包括生活型態問卷、體位測量包括身高、體重、腰圍、臀圍,並計算身體質量指數及腰臀比等;生化檢驗值包括總膽固醇、三酸甘油酯、高密度膽固醇、血尿酸、血糖等;動脈硬化測量則以非侵入性動脈硬化篩檢儀求得PWV。在除問卷內容不完整及資料有疏失遺漏者後,本研究分析樣本有321人(過重者140人,肥胖者181人)。過重者與肥胖者平均年齡分別為43.1及43.5歲,PWV值分別為1508.6及1545.1(公分/秒)。在多變項迴歸分析,BMI(β=-11.94,p<0.05)與PWV成負相關,收縮壓(β=7.87,p<0.001)、總膽固醇(β=0.98,p<0.01)和血糖(β=1.13,p<0.01)與PWV呈現正相關。在多變項邏輯斯迴歸分析,僅發現收縮壓(OR=1.05,95%CI=1.01-1.09)和血糖(OR=1.03,95%CI=1.01-1.04)愈高者,其動脈硬化指標(PWV>1400公分/秒)異常的危險性則愈高,且有統計上的顯著差異(p<0.05)。由本研究以非侵入性動脈硬化篩檢儀檢測結果發現,在控制年齡、吸菸狀況、喝酒狀況、心臟疾病紀錄及其他可能干擾變項後,收縮壓和血糖對動脈硬化(PWV>1400公分/秒)有顯著的預測能力。 |
英文摘要 | The purpose of this study is to evaluate the association of cardiovascular risk factors to arterial stiffness pulse wave velocity (PWV) among overweight Taiwanese men and to explore which cardiovascular risk factors are most associated with abnormal PWV. A total of 321 men (140 overweight men and 181 obese men) were included in this cross-sectional survey. A structured questionnaire was performed to evaluate lifestyle and disease history. Anthropometric measures were measured using standard method. Plasma choleserol, Triglyceride and Glucose level were measured using Olympus AU 600 by standard method. Non-invasive arterial stiffness measurement were measured using Colin VP-1000 by trained technicians. The mean ages of overweight and obese men were 43.1 and 43.5 years old respectively. PWV were 1508.6 and 1545.1(cm/t) respectively. PWV was positively correlated with SBP, DBP, HR, Cholesterol, TG and glucose level(r=0.17~0.56, all p<0.01). In multiple regression analyses, after adjusting for the potential confounders, SBP , cholesterol and glucose were still significantly associated with PWV. Using multiple logistic regression analyses, only SBP and glucose were significantly associated with abnormal PWV(OR=1.05 and 1.03 respectively, p<0.05). In this study, we used a non-invasive instrument to measure arterial stiffness and found that after adjusting for age, smoking, drinking and disease history, SBP and plasma glucose were the most significant factors associated with abnormal PWV among overweight Taiwanese men. |
