中文摘要 | 我們採用低頻低位聚合酶鏈鎖反應(infrequent-restriction-site polymerase chain reaction, IRS-PCR)的方法,分析三種Candida species 共25株菌株的分子生物分型,以了解其適用性,以及這些菌株彼此間的關係。分析的菌株中,包括來自3個病人的5株C. albicans和來自2個病人的8株C. glabrata,它們是分別來自不同新生兒加護病房的兩起「疑似院內血流感染群突發」的菌株。研究中並且加入由其他病房分離培養到的非院內感染菌株,包括3株C. albicans和4株C. glabrata,作為比較之用。另外,我們也分析了C. parapsilosis,包括5株院內感染菌株和3株非院內感染菌株,以了解該在院內萬播的情形。結果顯示,8株C. albicans得到6種分型,排除群突發的可能;12株C. glabrata得到5種分型,且其中8株院內感染菌株屬於同一分型,證明其彼此相關。8株C. parapsilosis的分型完全不同,表示並無交互感染的可能。由以上結果,我們證明IRS-PCR可以用作此三種Candida species的分型之用。以此方法的速度和通用性,它很顯然可以提供許多迅速可靠的資訊,幫助感染管制工作更有效的執行。 |
英文摘要 | Infrequent-restriction-site PCR (IRS-PCR) was adopted to fingerprint the three Candida species in order to confirm its applicability in molecular typing in our laboratory. A total of 28 isolates were included: (1) 5 strains of C. albicans and 8 of C. glabrata isolated from the blood of 3 and 2 patients, respectively, in two clusters of infections in neonatal intensive care units (NICU), considered to be 2separate outbreaks; (2) 3 stains of C. albicans and 4 of C. glabrata isolated from the wards at different instances; and (3) 5 nosocomialand 3 community strains of C. parapsilosis. Results showed that there were 6 molecular types for the 8 C. albicans strains tested, excluding the possibility for the presence of an outbreak. There were 5 types for 12 strains of C. glabrata, of which 8 isolates from the two patients in the NICU showed an identical result. As expected, all 8 strains of C. parapsilosis had different molecular types. IRS-PCR appears to be suitable for the typing of the three Candida species. The IRS-PCR can be performed relatively quickly. The method is useful in the investigation of nosocomial yeast outbreaks. |
