中文摘要 | 快速增加的高齡人口,加上疾病型態的慢性化,社會環境、家庭結構型態及傳統家庭倫理觀念的改變,使國人對長期照護的需求遽增;病患服務員(nurse aid)在醫療體系中為半專業人員,無專業執照,亦不具相關學歷認定,但經由政府或授權單位施以相關之基本技術訓練,經資格核可後而參與服務者;其主要工作職責為執行患者之各項日常生活與身體照顧服務等最基礎的照護工作,其在長期照護所扮演的角色日益重要,是目前政府解決失業問題的策略之一。 自91年8月5日至9月9日以結構式問卷調查臺北市立醫院之出院病患,由調查結果可看出臺北市立醫院的住院患者,其主要陪伴者仍以家屬(67.6%)為主,其次依序為夫妻(28.2%)、母子(11.5%)、母女(10.6%);且其中80.6%者願意委託他人照顧,可看出主要照顧者之疲累,故除了要提升患者的照顧品質外,更需關心主要照顧者之健康狀況。由於市民對市立醫院的信心,67%者放心將患者整個交給醫院照顧,83%願意付費,但多數65.5%希望採一位病患服務員照顧一位病患的方武,34.2%者同意可照護二位病患,故未來推行此制度時一位病患服務員照顧以不超過二名為宜,且病患的嚴重度相近,床位安排亦需靠近,不但可減輕家屬負擔,亦不致影響照護品質。 家屬多希望採三班制,每班600元;採二班制,每班1000元;採全日班則認為1900元合適;若採彈性上班,每小時100元較符合患者之需求。目前醫療機構中,病患服務員係由病患或家屬自行僱用,執行一對一的病患照顧工作,照顧費用每月約需6萬元,對長期照護病患之家庭而言,是一筆龐大數目,若能提供一位病患服務員照顧二位病患的服務模式,可減輕病患或家屬經濟負擔。 |
英文摘要 | With the increase of manpower and elevation of the quality of service for the residents and patients, it is necessary to understand patient's need in nurse-aides in various municipal hospitals. The exploration in this survey included the demographic data of the patients, and the opinion of hiring of nurse-aides. A structural questionnaire was designed and data were collected from August to September 2002. The results indicated that most care givers were married middle-aged women. Around two thirds of the care givers need nurse-aides' help activity in daily living. Most of the customers were satisfied with the service of the hospital workers. They prefer one nurse-aide taking care of one to two cases. With regard to customers' opinion with different types of care shift, those who are served by 3 shifts, the salary is preferably 600 N.T. per shift; served by 2shifts, 1000 N.T. per shift; serve whole day, the salary 1900 N.T. In addition, those are served by part-time, the salary is preferably 100 N.T. per hour. The recommendations were proposed to modify the managing policy of the nurse-aides. |
