中文摘要 | 一些非Hodgkin氏淋巴腺癌(NHL)發現有特定的染色體變異。這類變異多半不異藉例行的染色方法(G-banding)清楚辨識,但藉新近開發的「頻譜式染色體分析法」(SKY)可以達成。SKY分析法是一項新的螢光染色體檢查方法,借助對交技術(hybridization)及電腦影像分析系統(spectral imaging system)可一次將不同染色體以不同螢光顏色區分開來。本文報告一位七十一歲非本土性Burkitt氏淋巴腺癌女性患者,藉傳統染色體檢查得知,此患者之染色體出現複雜的變異情形,其中包括為2、3、6、8、9、14、18對及X性染色體都有問題,但此異常對臨床上的診斷及治療幫助有限。藉由SKY分析法,我們很清楚的知道每一條異常染色體的變異情況,包括在Burkitt氏淋巴腺癌患者所常見的第8對和第14對染色體間之平衡性轉性。本項研究印證了「頻譜式染色體分析法」這套技術在分析染色體複雜變異上相當精確。 |
英文摘要 | Some cases of non-Hodgkin's lymphoma (NHL) have clonal chromosomal abnormalities with specific or non-specific changes. Such abnormalities are often difficult to identify by conventional G-banding, but relatively easy to refine by spectral karyotyping (SKY). SKY is a new molecular cytogenetic method that is based on the hybridization of 24 fluorescently labeled painting probes to a chromosome spread, allowing the visualization of chromosomes in different colors in one experiment. We report a case of Burkitt's lymphoma with involvement of many interchromosomal changes in a bone marrow specimen. By conventional G-banding, the analysis showed multiple numerical and structural chromosomal aberrations involving 2p, 3p, 6q, 8q, 9p, 14q, 18q and X chromosome, but provides little information about interchromosomal changes. With further studies by SKY, we were able to refine all the aberrations, including a reciprocal translocation between chromosomes 8 and 14, which is a frequently observed chromosomal rearrangement in Burkitt's lymphomas. This study illustrated the powerfulness of SKY in the refinement of chromosome aberrations and the importance of accurate laboratory findings in assisting clinicians to make timely decision to for mulate adequate patient care regimen. |
