中文摘要 | 背景及目的:利用自動化多葉式準直儀的方便性執行治療,免除複雜計算及技術限制的優越性,作為臨床治療何杰金氏症的應用分析。 方法:使用Siemens PRIMUS直線加速器附加之多葉式準直儀(multileaf collimator, MLC),將大照野之斗篷照野以小照野之輻射劑量加成整合,考慮其接面劑量以及不同照野離軸校正因子,並與傳統大照野治療比較。 結果:以多葉式準直儀分段調控照野技術治療何杰金氏症的腫瘤治療體積較小,97%或95%的等劑量典線便可以將腫瘤涵蓋,而傳統的斗蓬照野技術,其98%等劑量曲線所包括的腫瘤治療體積較大。此外,由體積直方圖可看出,分段式照野技術可提高腫接受的劑量,且肺部劑量及脊髓(spinal cord)的劑量也會減少。 結論:簡單的分段治療強度調控觀念,在臨床技術上有機會可以增加患者腫瘤的局部控制率,減少射束強度在肺部區域的劑量,減少病患治療後的副作用。此外,多葉式準直儀對於醫事放射師的操作方便性,也是此計畫很殺一價值的改良。 |
英文摘要 | Background and purpose: We performed the first clinical study of the utility of dividing large treatment fields into multiple small subfields with the multileaf collimator (MLC), part of Siemens's PRIMUS linear accelerator, to facilitate the treatment of Hodgkin's disease. Methods: We pooled doses in multiple small fields and analyzed them with the MLC because of its easier dosimetry and treatment technique. We considered interface doses and off-axis correction factors, and compared them with conventional treatment methods. Results: Segmented modulation through the use of MLC fields treated a smaller tumor volume, which could be covered by a 97% or 95% isodose curve. The conventional mantle technique treated a larger volume in a 97% isodose curve. The dose volume histogram analysis showed that segmenting the field increased the irradiated dose reaching the tumor and decreased the irradiated dose reaching the lung and spinal cord. Conclusion: The method of simple segmented intensity modulation may increase local control of tumor growth and decrease posttreatment complications. The MLC was a convenient tool for the radiologic technician to operate, and was a valuable improvement in this project. |
