中文摘要 | 目的:對精神分裂症病患之幻覺經驗以現象學取向探討其描述性的精神病理學臨床表癥。 方法:針對診斷確定之150名急性期精神分裂症病患,以半結構性之「精神分裂症幻覺評估表」進行會談與紀錄。對聽幻覺之臨床表癥,以人口學及基本臨床變項,如性別、年齡、教育程度、生病年數、醫院別等予以分析。 結果:一、元素性聽幻覺的內容以「人的動作聲」最常見。語言性聽幻覺之內容以「單句」「與病患對談」「命令」最常見。二、元素性聽幻覺與語言性聽幻覺特徵相近。三、聽幻覺最常引發的情緒反應為「害怕」,聽幻覺引發之情緒與行為反應與特定聽幻覺內容相關,如思考成音 (audible thought)、批評 (voice commenting) 和數人談論病患 (voice conversing),而與一般人口學及基本臨床變項無關。四、精神科專科醫院病患比綜合醫院病患受到聽幻覺對日常生活的影響較大。女性病患比男性病患較常出現「批評」的聽幻覺。 結論:本研究結果擴展精神分裂症病患各層面之聽幻覺經驗(例如內容、物理特性、來源與情緒行為反應)的瞭解,此結果可為精神分裂症研究與臨床服務之參考。 |
英文摘要 | Objectives: To use a phenomenological approach to explore the descriptive psychopathological manifestations of auditory hallucinations in patients with schizophrenia. Method: A total of 150 schizophrenic patients consecutively admitted to acute psychiatric wards (70 from a general hospital and 80 from 2 psychiatric centers) were enrolled in the study. A semi-structured questionnaire for schizophrenic hallucinations was used to collect information about the content and characteristics of auditory hallucinations and the emotional and behavioral response of the patient to auditory hallucinations. The effects of personal and clinical variables such as sex, age, educational level, duration of illness, and setting of hospitalization (psychiatric center or general hospital) on the hallucinatory experience were analyzed. Results : The two most common contents of elementary hallucinations and verbal hallucinations were "the sound of human actions" and "voice commanding". The characteristics of elementary hallucinations and verbal hallucinations were similar. Emotional and behavioral response to verbal hallucinations were each correlated with specific psychopathology such as audible thoughts, voice commenting and conversing, but not with personal and basic clinical variables. Patients in a psychiatric center had auditory hallucinations with greater frequency, higher volume and greater obliteration of normal sounds. Female patients heard "commenting" hallucinations more frequently than male patients. Conclusion: The results of this study may help to clarify the nature of different dimensions of auditory hallucinatory experiences, such as origin, producer, content , physical characteristics and emotional / behavioral response, of schizophrenic patients . The study results indicate the need for practitioners to be aware of the rich phenomenology of auditory hallucinations in schizophrenia which may facilitate better doctor-patient relationships and quality of care. |
