中文摘要 | 進階心臟救命術(ACLS;Advanced Cardiac Life Support)教育課程的設立迄今已約30年,其間曾經數次的課程修定增補以提升教學訓練效果。ACLS課程制定時就有規定每2年應重新再認證一次,而有認證之課程内容與原先課程内容均相同,須為為2天,此種再認證方式對於在危重症醫療單位服務之醫護人員而言,反覆參加實覺枯燥無味,反而降低了這些資深人員的再認證意願,有鑑於此,美國心臟學會自1998年起開辦針對此等資深人員而設計之ACLS-EP (ACLS for Experienced Providers)並先後在美國佛羅里達州、科羅拉多州及猶他州等處完成試教,效果良好,隨即正式頒布課程。 ACLS-EP課程包括三大部份:第一部份為ACLS複習部份(為時1小時半),其中一半時間為BLS-AED技巧複習評測,另一半時間為ACLS執行人員心肺復甦重要步驟口述評測。第二部份為ACLS-EP課程的核心,包含4個輪換教育站,每站為時90分鐘,這4個教育站為:1.進階心血管及肺部急症;2.電解質異常;3.環境因素所致之急症;4.毒物學急症。第三部份為筆試(為時1小時)。ACLS-EP課程設立之目的,是希望能藉此課程,使在危重症單位工作之人員(所謂的ACLS資深人員)可以由參加此富有挑戰性的課程而使其ACLS完成可以獲得再認證之機會,對於不須取證的醫護人員,參加此課程也可以獲得參加危重症繼續教育的學分! ACLS-EP課程的設立是代表ACLS教育的新里程碑,期望能加速此課程的推廣,能更為提升ACLS資深執行人員對危重症病人的處置能力! |
英文摘要 | Advanced Cardiac Life Support Course (ACLS) for health care providers has been introduced into general health profession for about 30 years. Since its first introduction in the 1970's, the learning and teaching of ACLS has been revised for several times to facilitate maximal effect of ACLS education. The education of ACLS provider course requires that course completion card renewal every 2 years. For providers experienced in critical /acute/emergent, medical/nursing care), repeated exposure to the space general content of the ACLS provider course results in less of motivation for attendance and even renewal. This fact stimulated the design of the ACLS for experienced providers (abbreviated as ACLS-EP Course). The pilot courses were conducted in Utah in 1998. In 1999 the instructor's manual of the Advanced Cardiac Life Support for Experienced Providers was published. The ACLS EP Course takes one day to complete. It is divided into 3 parts: part 1. (Review part) (a) BLS-AED skills evaluation using Heartsaver AED checklist (45 minutes) (b) verbal assessment of critical ACLS provider actions (45 minutes). Part 2: (learning stations): There are 4 rotating learning station (each 90 minutes) A. advanced cardiovascular and pulmonary emergencies. B. Electrolytes abnormalities. C. Envinonmental emergencies. D. Toxicologic emergencies. Part 3: Written evaluation (60 minutes). The course is intended for experienced ACLS providers with valid ACLS provider card to renew their ACLS provider status through a more challenging 1 day educational course. For health care providers unwilling to renew their ACLS cards, through the ordinary ACLS providers' course, the course ACLS-EP can be a good choice for continuous medical education. It is hoped that through the development of ACLS EP course, a more in-depth education and training in critical/emergent illness management can be introduced to the experienced ACLS providers. |
