英文摘要 | Background: Decreased cerebral blood flow (CBF) is one major factor causing dysfunction of the central nervous system in heat stroke. Patients with heat stroke showed increases of blood cell counts and blood viscosity, which might aggravate cerebral ischemia induced by heat stroke. In the present study we used hemodilution therapy to the heat stress rats to determine 1) the alterations of local cerebral blood flow (LCBF) in heat stroke animal; and 2) any beneficial effects on heat stroke animals by the use of hemodilution. Materials and Methods: Sprague-Dawley rats weighing 250-400 gm (n=45) were divided into three groups. The first group (the control group) animals (n=15) were kept at room temperature (20℃). The second group (the heat stroke group) animals (n=15) were put in high temperature environment (42℃) until the onset of heat stroke. The third group (the hemodilution group) animals (n=15) were treated by hemodilution immediately after the onset of heat stroke. The hematocrit of the heat stroke animals were lowered down to about 29% by replacing blood with geloplasma. The LCBF of 14 brain structures were measured for each animal by the use of (superscript 14)C-iodoantipyrine technique. Results: The body temperature elevated from 37.8±0.5 to 40.9±0.6℃ after heat stress, and it dropped to 38.4±0.6℃ after hemodilution. In control group the LCBF at 16 brain structures were from 109.92±28.94 to 275.67±61.27 ml/100gm/min, whereas, the LCBF of the heat stroke group dropped from 67.28±4.70 to 118.82±4.56 ml/100gm/min. The LCBF of the hemodilution group were from 123.38±19.69 to 247.87±66.46 ml/100gm/min. Conclusions: In severe heat stroke patients, conscious disturbance was noted. Evidence show the reduction of the cerebral blood flow is related to the dysfunction of the brain. Our present study showed marked decrease of LCBF in heat stroke animals, and after hemodilution the LCBF returned to normal. It seems moderate hemodilution could possibly improve the outcome of heat stroke patients. |
