中文摘要 | 本回溯性的研究蒐集了33位臨床疑似嗜鉻細胞瘤的高血壓患者,分析其 I-131MIBG閃爍攝影,配合X光電腦斷層攝影(CT),以評估I-131 MIBG診斷此類患 者的情形。12例經手術切除及病理檢查證實為其正病灶,21例經由臨床追蹤或手 術排除嗜鉻細胞瘤或其他能產生catecholamine的腫瘤。12例真正病灶中,7例為為 單側腎上腺嗜鉻細胞瘤,2例為兩側腎上腺嗜鉻細胞瘤,1例為神經節神經性細胞 瘤,1例兩側腎上腺髓質肥厚症,1例為轉移性嗜鉻細胞瘤。MIBG均將這些病灶 顯影,CT則在MIBG的指引下,除了一例腎上腺髓質肥厚症無法確定診斷外,在 相關部位的檢查也將病灶很好的定位。我們認為MIBG檢查是一個敏感度很高的 檢查,除了腎上腺外,也可以偵測異位性的嗜鉻細胞瘤或其他神經內分泌瘤。若 配合CT檢查,則可以很好的替病灶定位。(慈濟醫學1994:6:41-48) |
英文摘要 | In this retropective study, we performed I-131 MIBG scintigraphy and X-ray CT in 33 patients with clinicalsuspicious of pheochromocytoma. 12 cases were proved to have true organic lesion by surgery and pathology, 21 wereruled out through long-term clinical follow-up and operation. Among 12 cases of adrenal medullary lesions, 7 caseswere unilateral adrenal pheochromocytoma, 2 cases were bilateral pheochromocytoma, 1 case was ganglioneuroma,1 cases was bilateral medullary hyperplasia, 1 case was metastatic pheochromocytoma. 1-131 MIBG demonstratedall these lesions. Under guidance of MIBG, CT well localize all lesions except the case of adrenal medullary hyperplasia. We conclude that, I-131 MIBG is a high sensitive modality for detecting catecholamine secreting lesions.Beside adrenal gland, it can also detect ectopic pheochromocytoma or other neuroendocrine tumors.
