英文摘要 | Objective: This is a retrospective study to evaluate the clinical outcome in patients who underwent segmental pedicle screw hook fixation to repair their pars defect. Methods: Fifteen patients were included in this study (From Aug. 1997 to Aug. 2000). They all suffered from lumbar spondylolysis with either a grade 0 or less than grade 1 spondylolisthesis. The average age at the time of surgery was 22.8 (range: 19 to 26 years). They were all young males previously engaged in heavy manual labor. One surgeon performed the operation, and a follow-up was conducted on all patients for an average period of 20.4 months. Results: The clinical results were evaluated by Henderson's categories of functional capacity. The preoperative evaluation revealded "fair" in 10 patients, "poor" in 4 patients and "good" in 1 patient. Postoperative evaluation revealed "excellent" in 12 patients and "good" in 3 patients. Sequential X-ray follow-up showed a solid fusion of the pars defects in all patients (100%) as compared to what had been observed in the preceding X-ray check-up. In nine patients, removal of the implant was requested on year after operation. The fracture was examined during implant removal, and the examination showed a large bone mass in the pars defect region, and that the spinal integrity was preserved without pseudoarthrosis. Conclusions: This technique is a simple and effective method for the direct repair of spondylolytic defects. However, it is only indicated in individuals of yound age with pure spondylolysis or minor degrees of slippage. |
