中文摘要 | 一位工業輻射技術人員在偵測油槽焊接關節時,意外暴露於銥-192射源高劑量的游離輻射。手部接受劑量約為52Gy,全射接受量約為2-4Gy。三天後,病人出現雙手手指脫皮、紅腫、壓痛、潰爛現象,此病徵約20天後消失。半年後,雙手手指又出現紅腫、脫皮並慢慢有潰爛現象。病人至地方診所尋求治療,診所給予換藥治療,但患都手指情況持續惡化,局部疼痛加劇、潰爛範圍增大,最後逐漸軟組織壞死、指引脫落、指骨露出。因病情持續進展,此病人在意外發生十八個月後約每隔六個月接受右手第二、三指及左手第二、三指截肢皮皮瓣重建。在接受最後一次截肢手術後,病人開始口服血液循環促進劑及使用超氧化物歧化脢(SOD-superoxide dismutase)局部塗抹患處,一天二至三次,持續至今約半年。使用SOD前的皮膚生理檢查顯示引床微血管數目降低並扭曲、流速減緩,指溫降低,在使用SOD後追蹤的皮膚生理檢查則顯示甲床微血管數目略為增加、扭曲程度趨緩、流速及皮膚血流灌注量逐漸趨於正常。臨床上,病人的痛感及促膚病變也有顯著改善。 |
英文摘要 | An industrial radiographer was accidentally exposed to a high dose of ionizing radiation from an Ir192 source during radiography of weldjoints in oil pipelines. Skin erythema, swelling, ulceration developed within 3 days, leading to progressive tissue deterioration. The dose to the whole body was estimated to be about 2-4 Gy, and the dose to the fingertips was approximately 52 Gy. He visited a medical practitioner who approached conservatively including local skin care with dressings. The wounds of the fingers still deteriorated progressively. Surgical amputation of the second and third fingers of right hand was performed after 18 months. The secnd and third fingers of left hand reached the same destination 6 months after the amputation of the contralateral side. The hand of the patient appears with xerosis, telangiectases, hyperpigmentation, atrophy, and bone exposure, especially the right thumb. After amputations, the patient applied S.O.D. on the fingers twice a day for 6 months. We found that there were increase in the number and the velocity, but decrease in the tortuosity of the capillaries on nail folds. The skin perfusion by laser Doppler flowmetry tended to normal level. Clinically, the pain sensation and the skin lesions improved. |
