中文摘要 | 膀恍內輸尿管射尿可以藉彩色都卜勒超音波術加以觀察,其動力學變化並可由 都卜勒波形的頻譜分析來呈現。在充分的給水狀況下(例如:於30分鐘內喝下1,000ml左右 的無糖烏龍茶),都卜勒的觀察顯示:正常輸尿管射尿波形為多波峰狀;在腎泌尿的利尿期 (renal diuresis)內,健常者單側的每次射尿平均速度(Vmem)為≧2Ocm/sec,每次射尿時 程(D)為≧4.5sec,並且每10至3Osec射尿1次(t=l0-3Osec,F=l/10-3Osec)。若以Vmem( cm/sec)╳D(sec)╳F(sec-1),則可求得輸尿管射尿指數(uretericjet;ndex,UJI),其正 常值為單側≧lOcm/sec和兩側≧2Ocm/sec。在腎利尿期之下,腎泌尿扮演著輸尿管射尿的 主要動源,因此輸尿管射尿指數實際上是腎功能的一項指針。除外,藉輸尿管射尿的都卜 勒波形之觀察,發現尿路阻塞時多峰的波形會改變,而形成3種類型:(1)單峰型、(2)扁 平型和(3)無射尿;前2者表示不完全阻塞,後者表示完全性阻塞。輸尿管射尿的都卜勒頻 譜分析,有一個最重要的好處,即:可以單側性地觀察輸尿管動力學變化與腎泌尿功能。 側性地觀察輸尿管動力學變化與腎泌尿功 能。 |
英文摘要 | The ureteric jet can be visualized by color Doppler ultrasound, its dynamic observation can be achieved with spectral analysis of Doppler waveforms°]DWs°^. The DWs are normally multi-peaked if well hydrated; in the phase of renal diuresis the average velosity °]Vmean, cm/sec°^ of urine ejaculation for each jet is normally 20cm/sec, the duration°]D, sec°^ 4.5sec and its frequency°]F, l/t, sec-1°^ one ejaculation per 10 to 30 sec°]t = 10 to 30sec°^. Ureteric jet index°]UJI, cm/sec°^is calculated through Vmean° ]cm/sec°^╳ D°]sec°^╳ F°]sec-1°^;normally the UJI is 10cm/sec unilaterally and 20cm/sec bilaterally. Since renal diuresis is the toggle mechanism of ureteric jet, UJI is clinically an index of renal function. In cases of obstructed uropathy the DWs could be°]1°^monopeaked, °]2° ^flattened or even°]3°^no jets°^ instead of multipeaked; the former 2 mean incomplete obstruction and the last one means complete obstruction.
