中文摘要 | 基於疾病早期診斷早期治療的理念,本研究之目的在評估高壓氧治療對早期糖尿病足患者血管功能的改善效應。本研究從民國88年9月至89年8月,採隨機分組將研究對象分為治療組與控制組兩組,共追蹤治療組14位接受血糖藥物控制、潰瘍傷口清創及輔助性高壓氧治療病患;而12位控制組病患除未接受高壓氧治療外,其餘同治療組。實驗結果顯示:接受高壓氧治療病人的白血球在治療後比治療前低(p<0.05);於傷口邊緣或足部趾端,組織氧濃度於第三十次高壓氧治療後有顯著增高,且與本身治療前相比亦有顯著增高的現象;而足部血流掃瞄顯示高壓氧治療後,治療組的足部血流比控制組的足部血流多。本研究結果顯示高壓氧治療在改善早期糖尿病足之足部組織氧濃度、足部血流量等具有臨床重要效性。 |
英文摘要 | Twenty-six patients with diabetic foot were selected from Tri-Service General Hospital between September 1999 and August 2000. Fourteen and twelve patients were assigned to treated and controlled groups, respectively. Patients in the controlled group were treated like the patients in the experimental group in every ways except they did not receive the hyperbaric oxygen therapy (HBO₂) intervention. Blood biochemical examinations showed the patients in the treated group had significantly lower white blood cell counts after HBO₂ treatment. The treated group had significantly higher transcutaneous oxygen tension and higher blood flow of foot than the controlled group after the 30th course of HBO₂ treatment. HBO₂ can promote the vascular function, avoid the pathologic change, and further prevent the damage, ulcer, and infection of diabetic foot. In addition, HBO₂ can help the wound healing, prevent severe tissue necrosis, and avoid lower limb amputation. |
