英文摘要 | Carcinoembryonic antigen (CEA), CA125, and CA19-9 are well-known tumor-associated markers, however, they are not specific for malignancy nor for tumor type. They can be detected at a high serum level in some benign diseases, such as in active interstitial lung disease. We report two cases of idiopathic interstitial pneumonial (IIP) found to have high levels of serum tumor markers, one with elevated CA125 and CEA and the other with elevated CA125 and CA19-9. The two patients were admitted on account of severe dyspnea and hypoxemia, and their chest radiographs showed diffuse interstitial lung infiltrates. During admission, a series of studies failed to find any malignancy, and rapid clinical deterioration was accompanied by a progressive elevation of the serum tumor markers despite high dosages of steroid treatment. The mechanism involved in elevating serum tumor-associated markers in patients with IIP is not yet clear. The tumor-associated antigens, including CEA, CA125, and CA19-9, can be detected in apparently normal lung tissues. It has been postulated that persistent epithelial cell damage in the lower respiratory tract may alter the barrier between the airway mucosa and the circulation, increasing the permeability of the peripheral respiratory epithelium, and thus resulting in increased serum tumor-associated antigens in patients with IIP. A high serum level of these antigens is not indicative of pre-cancerous alterations, instead, it may reflect the activity (severity) of IIP. |
