中文摘要 | 阻塞性小支氣管炎及組織化肺炎是一侵犯小支氣管及肺泡的炎性疾病。典型表現在症狀上似一般肺炎而胸部影像為下肺部邊緣性實質化。一般來說,此類病人對類固醇療效佳,但仍有其他因素影響其預後,在分類上,不明原因性的仍佔大部分,而部份病人合併其他狀況,如藥物、膠原性疾患、放射治療、感染或器官移植。在此,我們報告一病例為阻塞性小支氣管炎及組織化肺炎合併類風濕性關節炎,其胸部X-光以不尋常的上肺葉侵犯來表現。此病人雖經類固醇治療仍演變變成急性呼吸窘迫症。有趣的是,病人正好此時亦發生急性?咯紫質沉著症。我們也試圖從文獻中尋找此病和上述病症之相關性。 |
英文摘要 | Bronchiolitis obliterans-organizing pneumonia (BOOP), or cryptogenic organizing pneumonia, is an inflammatory disease of the small bronichiole accompanied by an excessive parenchymal involvement of the lung. The patients typically present with flu-like symoptoms, and the chest roentgenogram reveals peripheral conslidation in the lower lung fields. Generally, patients with BOOP have a good response to steroid therapy and a better prognosis than those with bronchiolitis obliterans or pulmonary fiberosis. The major cause of this disease is idiopathic. Some factors influence the outcome of BOOP, such as rapid progression and the failure of conventional treatment. Herein, we report a case of BOOP associated with rhenumatoid arthritis, which demonstrated an unusual presentation of bilateral upper-lobe opacities on the chest X-ray. The patient also presented with acute neurovisceral prophyria, an inherited metabolic disease. The patient had an atypical course, with rapid exacerbation to respiratory failure in spite of steroid therapy. We will also explore the possible correlation between BOOP and porphyria. |
