中文摘要 | 背景:聲門動態變化是喉生理學研究的一個重要課題,然而各個研究工具在使用上仍有其限制性。本研究嘗試結合喉頻閃鏡和聲門光圖檢查所能呈現的兩種影像來研究音調變化時聲門面積之動態變化,藉此評估此種觀察模式之可行性,並了解在不同音調時聲門之形態變化。 方法:研究對象為20名正常成人,同時接受喉頻閃鏡和聲門光圖檢查。我們將喉頻閃鏡之閃光和聲門光圖檢查之持續光的強度做適當的調整,同時呈現動態之聲帶振動及最小聲門影像。觀察研究對象在最舒適之發音強度下以較低音調、正常音調以及較高調持續發母音 /a/ 時之聲帶振動影像。 並以數位影像分析軟體測量聲帶振動振幅、膜部聲門寬度、後聲門寬度以及聲門變動面積等影像參數,結果再以Mann-Whitney Rank Sum Test統計方法分析影像參數於各音調間之差異性。 結果:我們成功地觀察受試者在不同音調時聲門之形態變化。聲帶振動在音調較低時振幅較大,而在音調較高時則振幅較小。膜部聲門寬度在音調較低時較小,而音調較高時則較大。後聲門寬度雖於不同音調時差異性不明顯,但仍呈現有低音調時較大,高音調時較小的趨勢。聲門變動面積則在較低音調時變化較大,而在較高音調時較小。 結論:本研究將喉頻閃鏡和聲門光圖檢查光源所能呈現的兩種影像結合在一起,並成功地運用於研究音調變化時聲門變動面積之變化。本研究所提出新的影像參數以及使用的觀察模式提供了進一步研究聲門動態變化的另一種選擇。 |
英文摘要 | BACKGROUND:The dynamic change of glottal area is a major concentration in the field of laryngeal physiology; however, the individual investigating tool has its own limitations. The current study attempted to combine the image of stroboscopy with photoglottography to investigate the effect of changes in pitch on the dynamic change of glottal area. The aim was two fold: first, to evaluate the applicability of this new model of observation, and, second, to determine the influence of pitch on the dynamic change of glottal area. METHODS: Twenty adults were enrolled in the study and simultaneous stroboscopy and photoglottography were applied. The intensities of strobe light and constant light were adjusted to obain an image imcorporating both dynamic change and unchanged minimal glottal area. Images were recorded and digitized during phonation of the sustained vowel /a/ at a comfortable intensity for each subject's lower, normal, and higher ranges of pitch. Images parameters, including amplitude of vocal fold vibration, membranous glottal width, posterior glottal width, and dynamic glottal area, were measured and analyzed with digital image analysis software. A Mann-Whitney Rank Sum Test was used to identify significant differences of image parameters between pich groups. RESULTS:The glottal configuration was successfully observed at different ranges of pitch. Amplitude of vocal fold vibration was found to be larger at lower pitch and smaller at higher pitch, with membranous glottal width smaller at lower pitch and larger at higher pitch. The differnces of posterior glottal width between pitch groups were not significant; however, a tendency of being larger at lower pitch and smaller at higher pitch was identified. The results also demonstrated that dynamic glottal area was larger at lower pitch and smaller at higher pitch. CONCLUSIONS: This study combined the image of stroboscopy with the image produced by constant light for photoglottography and was successfully applied in the investigation of the effects of changes in pitch on the dynamic change of glottal area. These new image parameters, and the model of observation, provide an alternative method for further study of the dynamic change of glottal area. |
