英文摘要 | An endogenous factor released from ischemic hindlimb increases the sensitivity of calcium channels, thus may contribute to increase vascular smooth muscle contractility and post-operative cardiac arrhythmia in infra-renal aortic cross-clamping. Treating hindlimb ischemic rats with nifedipine may increase the survival rate by inhibition the hyperactivity of calcium channels in cardiac myocytes and vascular smooth muscles. Hindlimb ischemia was generated in rats by infrarenal aortic cross-clamping for five hours after which the clamp was removed. Ischemic rats showed a 50% mortality two hours after declamping. The calcium channel antagonist, nifedipine, significantly inhibited the arrhythmic rates two hours after declamping. In vitro incubation of naive aortic rings (endothelium removed) for two hours in plasma collected from the femoral vein of ischemic rats demonstrated a significant decrease in the concentration of the calcium channel agonist, Bay K 8644, necessary to produce the threshold (EC[8e45]) and half maximal (EC[9274]) responses when compared to the response of control-plasma incubated rings (EC[8e45]=2.5 nmol/L and EC[9274]=10 nmol/L for ischemic-plasma incubated; EC[8e45]=7.9 nmol/L and EC[9274]=25 nmol/L for control-plasma incubated). After ischemic-plasma incubation, the washout curve following contraction to phenylephrine was not significantly altered. However, when rings were incubated in ischemic plasma, the washout curve after contraction to Bay K 8644 in either a calcium enriched or calcium free solution was prolonged. Prolonged increase in the sensitivity to Bay K 8644 suggests that an endogenous factor from the ischemic hindlimb may contribute to post-operative mortality of ischemic rats by increasing myocardial irritability and decreasing coronary blood flow. |
