中文摘要 | 目的: 精神疾病覺知障礙量表(SUMD)係一多層面病識感之評量表,評量個案是 否具有:1.罹患精神疾病的覺知,2.藥物治療效果的覺知,3.精神疾病造成之社會影響的覺 知,4.對於精神症狀的覺知,5.對其精神症狀做正確歸因。本研究嘗試完成精神疾病覺知障 量表翻譯與中文版修訂,檢驗其信、效度,並探究精神病患缺乏病識感的臨床相關因素。方 法:研究樣本來自北部某醫學中心的精神科急性與復健病房:針對精神分裂症患者,進行量 表之再測信度與評分者間信度檢定;在量表之效度檢定方面,以中文版活性與負性症狀量表 中的G12得分進行幅和效度的檢定。結果:本量表之再測信度及評分者信度均相當穩定。量 表中的3題一般題項與兩個次量表與G12得分均呈現有意義的相關;活性量尺、負性量尺、一 般精神病理及總分與SUMD無顯著相關。教育程度愈高、發病年齡愈晚,其對精神疾病的覺知 能力也愈差;愈處於急性期的個案對藥物治療所獲致效果的覺知也愈差。結論:此一多層面 評估精神分裂症病患病識感之量表,有效評估病識感之多層面,將可提供臨床相關領域的參 考與運用,而有利於醫療策略之擬定。 |
英文摘要 | Objectives: The "Scale to assess Unawareness of Mental Disorder" (SUMD) is a multidmensional scale used to evaluate a patient's awareness of their mental disorder, the need for treatment, the consequences of the mental disorder , and the awareness of and attribution of specific signs and symptoms. This study evaluated the reliability and validity of a Chinese SUMD version in the clinical setting and explored clinical correlates of poor insight. Methods: The study subjects included patients with schizophrenia admitted to the acute psychiatric ward and rehabilitation ward of a medical center. Patients were interviewed to confirm inter-rater and test-retest reliability and the G12 item of PANSS (Positive And Negative Syndrome Scale) was correlated with the SUMD score to verify convergent validity. Results: The Chinese SUMD version had good inter-rater and test-retest reliability. Convergent validity was also good through five dimensions. The positive syndrome, negative syndrome, general psychopathology and total scores in PANSS did not correlate with the SUMD subscales. Level of higher education and late disease onset correlated with poor awareness into their mental disorder; while patients in acute phases showed poor awareness of the need for treatment. Conclusion: The Chinese SUMD version was a reliable and valid scale for the evaluation of multiple dimensions of insight in schizophrenia. It lends itself to clinical use and can be used to guide further management strategy. (Full Text in Chinese) |
