英文摘要 | Purpose of Study:Cubitus varus is one of the most common complications following supracondylar fracture of humerus in children, and the recurrence after surgical correction remains to be a difficult problem. This is a retrospective study to evaluate our clinical experiences, and to identify the principal factors for the recurrence. Materials and Methods: 52 patients with 53 elbows receiving humeral supracondylar closing wedge corrective osteotomy with crossed or parallk Kirshner wire fixation from 1989 to 1998 were retrospectively reviewed. The humeral-elbow-wirst (HEW) angle, baumann’s angle, and carrying angles were evaluated for an average follow-up period of 14.7 months. Results: 15 patients (28.3%) lost correction after surgery, including those 5 patients who had elbow instability preoperatively. Though not statistically significant, the averae duration from injury to operation seemed to be longer for those patients who had lost of reduction, specially for those with elbow instabilities. Furthermore, the lost of reduction generally took place intraoperatively, since we noticed that the Baumann’s angle was already lost in the first follow-up. Conclusions: Ther recurrence of cubitus varus after surgical correction could be due to undercorrection, or unstable internal fixation. Prolonged period of deformity may further lead to soft tissue imbalance and probably articular structural change that would influence the operator’s judgement during percutaneous fixation of the Kirschner wires. |
