中文摘要 | 鑑於智能不足兒童之早期發展與智能之關連尚不十分清楚.本研究長 期追蹤47位純由智能不足導致身心發展遲緩兒童之發展,並探究其早期之動作 發展、發展評估結果、行為表現與智力之相關,以作為醫護人員,病童家屬與特 殊教育人員輔導或治療之參考。由1986年12月到1988年8月間,在臺大醫院 復健科長期追蹤102位非唐氏症身心發展遲緩兒童中,選取非特異性智能不足兒 童,共47例的基本資料與發展資料納入分析。開始評估時年齡為4-42個月,平 均追蹤30個月。這些智能不足兒童之智能,根據「貝萊氏嬰兒發展量表」(Bayley Scale of Infant Development)之心理量表智能發展指數(mental developmental index, 簡稱MDI)或發展商數之結果,64屬重度智能不足(發展商數<35),32屬中度智能 不足(發展商數>35,且MDI<50),僅有4屬輕度智能不足(MDI 50-70)。結果顯示 早期之各項發展評估與後來智能皆有關連。在早期臨床評估各變數中,以「早期 介入發展量表」之精細動作發展商數、有無固定性動作行為、及「學齡前兒童發 展量表」中之一般發展發展商數與智能不足兒童智能之相關性較高。由於本研究 平均只追蹤到53個月,因此尚不能確切代表未來之智能功能。而且智能不足嚴 重度之判別,除智能發展商數外,也可用適應行為之表現來判定。所以本研究之 結果,可作為臨床諮詢與治療之參考,卻不宜作為判定未來智能不足嚴重度之標 記。(中華物療誌1994;19(1):29-37) |
英文摘要 | The psychomotor development of mentallyretarded children has seldom been studied inTaiwan. To find out the associated variables oflater mental outcome of non-specific mentallyretarded children, 102 neurologically normal,non-Down's psychomotor retarded children, hadbeen recruited and followed from December1986, till the age of 2-8 years old. The durationof follow-up is 9 to 48 months (mean 30months). Among them, a total of 47 children,with the final diagnosis of non-specific mentalretardation, were analyzed in the resultssection.Their final intellectual functions, represented by mental developmental index (MDI) ordevelopmental quotient (DQ) of mental scale ofBayley Infant Developmental Scale, weremostly among moderate to severe retardation. Sixty-four percent of them was severe(DQ <35), 32 moderate (DQ > 35 and MDI < 50),and 4 mild (MDI 50-70). The variables of initial evaluation, which were able to predict thefinal intellectual functions, included fine motorDQ of Early Intervention Developmental Profile, existance of stereotyped movements,andDQ of General Development of Chinese Children Developmental Profile. Therefore, DQs ofthe initial evaluation of mentally retarded children could be used for predicting later mentalfunctions, guiding treatment of these childrenand counselling their parents. However, clinicians should also look for the adaptive skills ofthe children to prevent early mislabelling.(JPTA ROC 1994;19(1):29-37) |
