中文摘要 | 高濃度側鏈氨基酸(branched-chain amino acid,簡稱BCAA)靜脈輸輸液,如Aminosteril N-Hepa,因BCAA可在肝外細胞代謝,臨床上可增加氨基酸利用率及減輕肝細胞負擔。為評估 Aminosteril N-Hepa在小兒外科病患之臨床效果,本計劃於臺大醫院小兒外科病患,接受全靜脈營養超過10日者30名,隨意性分為兩組(每組15人),分別給予等量 8% Aminosteril N-Hepa(含42% BCAA)及12% Proteamin(含21%BCAA)氨基酸輸液( 1.5g╱kg╱day),其餘之葡萄糖(10g╱kg╱day)及脂肪乳劑(1g╱kg╱day)之給予則兩組間均相同。除詳細記錄其基本資料外,在使用前及使用後第3,7,10日測量:(1)白血球數、淋巴球數、血球比容及血紅素;(2)血清前蛋白、攜鐵蛋白、白蛋白、三甘油脂及膽固醇之濃度,血清各項肝功能(包括ALT、 AST、膽紅素、ALP、球蛋白)、腎功能(包括 BUN、creatinine )等指標及電解質(包括Na、K、Cl)含量;(3)血清氨基酸濃度分析;(4)尿液肌酸酐及3-methylhistidine 含量;(5)尿液氮素含量及計算氮素平衡;並觀察(6)使用期間各種反應及副作用記錄等。試用結果發現:(1)使用 Aminosteril N-Hepa可使用血清BCAA濃度及BCAA╱TAA(total amino acids,氨基酸總數)比值有意義上升(P<0.05 );(2)使用 Aminosteril N-Hepa可使氮素平衡由負值變為正值,有意義上升,且優於Proteamin組病患(P<0.05);(3)使用 Aminosteril N-Hepa之病患,尿中 3-methylhistidine之濃度有意義下降(P<0.05);(4)兩組病患之血清TAA 濃度均有上升傾向,但只有Aminosteril N-Hepa組有統計上差異(P<0.05);(5)兩組病患在試用期間,均無副作用產生。試用結果顯示,高濃度 BCAA 靜脈輸液在小兒外科病患術後全靜脈營養使用中,是一種值得肯定的氨基酸輸液配方。 |
英文摘要 | High branched-chain amino acid (BCAA) solution, such as Aminosteril N-Hepa, can be metabolized in extrahepatic cells and easily utilized. This study evaluated the utilization of Aminosteril N-Hepa in pediatric surgical patients, and compared the metabolic and nutritional effects with conventional amino acid solution. Thirty pediatric surgical patients were enrolled in this study. Amino acids with Aminosteril N-Hepa (42% BCAA solution) or Proteamin (21% BCAA solution) were randomly given at a dosage of 1.5g/kg/day to each group of patients (n=15). The total parenteral nutrition (TPN) also included glucose (10g/kg/day) and fat emulsion (1g/kg/day), and these were the same in both groups. On days 0,3,7 and 10, we measured: (1)WBC, symphocytes, hematocrit and hemoglobin; (2)serum nutritional indicators prealbumin, transferrin, albumin, triglyceride and cholesterol), liver function indicators, renal function indicators and electrolytes; (3)serum amino acids; (4)urinary creatinine and 3-methylhistidine; (5)nitrogen balance; and we also recorded (6)general response and side effects if any. We found that: (1)the Aminostril N-Hepa group had a higher mean serum BCAA level and a higher mean BCAA/total amino acid (TAA) ratio on day 10 (p < 0.05); (2)the Aminosteril N-Hepa group had a better nitrogen balance on day 7 and day 10(p<0.05); (3)the urinary 3-methylhistidine concentration decreased significantly in the Aminosteril N-hepa group on day 7 and day 10 (p < 0.05); (4)the serum TAA concentration tended to increase in both groups, however, it was statistically significant only in the Aminosteril N-Hepa group; (5)no side effects were noted in either group of patients during this study. BCAA enriched solution is recommended for TPN administration in pediatric surgical patients. |
