中文摘要 | 假使結腸內的壓力一定,則結腸直徑最大時緊張力最大。但是事實上有些病例的結腸直徑不大時就破裂了。本研究的目的是探討在結腸的壓力及緊張力和結腸的直徑之間是否有關聯。從民國八十年五月至民國八十五年四月,去除了有明顯腸道破裂、腹膜炎,一般情況很差,及無法得到壓力測量器的患者,共有三十位結腸癌併急性結腸阻塞的病例在手術中測量了盲腸的直徑及壓力。盲腸的直徑至少有7公分,壓力由2至26毫米汞柱,結果發現結腸壓力及緊張力並不一定隨著直徑的增加而昇高。造成的原因可能是有些阻塞發生得很快,或是迴盲瓣的閉鎖不全,或是有些患者併有麻痺性阻塞。由此研究認為結腸的壓力及緊張力不絕對隨著結腸直徑改變,這也可以解釋為何一些直徑雖小但是緊張力高的結腸為何破裂。 |
英文摘要 | Background: Given an equal pressure throughout the colon, the greatest tension will occur in the portion of the colon with the largest radius. But in reality, some colons, even with small diameters, perforated. Aim:To find whether there is a correlation between the diameter and the pressure and tension of the colon. Method: from May 1991 to Arp. 1996,30 patients with acute obstruction of colon caused by carcinomas had measurements of the diameter of cecum, and the pressure of colon. Patients who had frank perforation, peritonitis, and poor general condition were excluded. Result: The cecum were at least 7 cm in diameter, and the pressures ranged from 2 to 26 mmHg. It was found that the increase in diameter of the colon was not necessarily associated with the increase in pressure and tension. This may be due to faster obstruction, incompetence of the ileocecal valve, and presence of paralytic ileus in some patients. Conclusion: The pressure or tension of the colon dose not vary directly with its diameter; perforation of the colon is independent of the diameter but dependent on the tension. |
