中文摘要 | 慢性腹膜透析(PD)病人中之高通透性者常伴隨較高死亡率,本研究想探討這類病人的臨床特性。收集本院接受PD之180病人(男84女94,平均年齡:53±15)之初次腹膜平衡功能來做分析。多變數分析中發現4(superscript th hour D/P Cr ratio與病人的年齡,男性有相關,又以單變數分析發現4(superscript th hour D/P Cr ratio與年齡,男性,及糖尿病有關。比起年老病人(後四分之一),年輕病人(前四分之一)有較高之血中及透析液中的肌酐酸濃度。老年病人的4(superscript th hour D/P Cr ratio 比值較高(p=0.019),同時糖尿病的比例較高(15.6% vs. 2.2%,p<0.001)。全體男性病人比女性病人有較高的4(superscript th hour D/P Cr ratio (p= 0.003)。但排除糖尿病後,非糖尿病人的男女性別間則無此差異性。整體糖尿病人年紀較高,男性比例較高,同時也有較高的4(superscript th hour D/P Cr ratio.而且4(superscript th hour D/P Cr ratio與血中白蛋白有負向的線性關係(r=0.27, p=0.0012)。我們結論:高通透性者與老年,男性及糖尿病有關。這三種臨床特性導致這類病人有較高的死亡率。 |
英文摘要 | High peritoneal transporters (HT) are associated with higher mortality in chronic peritoneal dialysis (PD) patients. The aim of this study is to evaluate the clinical characteristics of HT. A total of 180 patients (86 males and 94 females) receiving chronic PD from November 1992 to March 2001 at our center were enrolled in this study. The patients’ mean age was 53±15 (13-90 years). As determined by multivariate analysis, the ratio of the 4(superscript th) hour dialysate/plasma of creatinine concentrations (4(superscript th) hr D/P Cr) was significantly related to the patients' ages (p=0.00l) and gender (male) (p=0.002). Univariate analysis also revealed that advanced age (p—0.00J), male (p=0.004), and diabetes (DM) (p=0.012) correlated with 4(superscript th) D/P Cr ratio. Younger patients (youngest quartile) had higher levels of 2(superscript nd) hour serum creatinine concentration (Cr) (p<0.00l) and 4(superscript th) hour dialysate Cr (p<0.001) than the older patients (oldest quartile). The younger patients also had lower levels of 4(superscript th) hr DIP Cr (p=0.019) and a lower percentage of DM (2.2% vs.15.6%, p<0.001). With similar ages and PD durations, the ratio of 4(superscript th) hr D/P Cr of the males was significantly higher than that of the females (p=0.003), but no gender difference was detected in this ratio of non-diabetes. The percentage of DM was significantly higher among males (36%vs.16%, p=0.002). Compared to non-diabetes, diabetes were older (p<0.001), and male preponderant (p=0.002), and had higher 4(superscript th) hr DIP Cr (p=0.0l6). The 4(superscript th) hr D/P Cr was negatively relevant to patients serum albumin concentration (r=-0.27, p=0.00l2). The high transporters had significantly lower serum albumin concentrations than the low transporters (p=0.036). In conclusions we found that high transporters are related to older age, male gender and DM. All of these factors carry higher cardiovascular co-morbid conditions, and account for a high mortality rate among high transporters in chronic PD patients. |
