中文摘要 | 爲了研究小兒中樞神經系統感染時cytokine是否是一個良好的指標,以及是否可經由cytokine的指數來鑑定病人感染的嚴重度,並區別是否爲細菌性腦膜炎,進而研究cytokine在中樞神經感染所扮演的角色,我們收集了42個中樞神經系統感染的病童;包括11人是腦炎,及11人爲非中樞神經系統感染者爲對照組,測定其在急性期及一部分恢復期腦脊髓中Interleukin-6(IL-6),Interleukin-8(IL-8)及Tumor Necrosis factor-α(TNF-α)的濃度,並和其他中樞神經系統感染的指標,如:發炎細胞數、糖分、蛋白質、免疫球蛋白G及白蛋白比率做比較,並檢討其和預後的關系。我們發現CSF中IL-6,IL-8及TNF-α的濃度在細菌腦膜炎的病人組,明顯的高於無菌性腦膜炎、病毒性腦炎及對照組,統計學上有顯著差異,與對照組比較下,其p值分別是p<0.005,p<0.005及p<0.005。而無菌性腦膜炎組,其CSF中IL-6,IL-8及TNF-α的濃度也明顯高於對照組,具統計學上之差異,其p值分別是p<0.005,p<0.005及p<0.005;而腦炎的病人,其IL-6,IL-8及TNF-α在細菌性腦膜炎及無菌性腦膜炎病人中,治療前後的差異,發現CSF IL-6及TNF-α的濃度在細菌性腦膜炎病人組治療後有明顯降低,而CSF IL-8在細菌性腦膜炎病人組及無菌性腦膜炎組在恢復期也有顯著降低,其p值分別是p<0.05及p<0.005。另外,IL-6,IL-8及TNF-α互相之間及對其他的發炎指數,如:CSF白血球數目、蛋白質、糖分、IgG level,index互相之間的相關性來比較,除了CSF中TNF-α及IL-6有相關趨勢外(r=0.64,p=0.08),其他並無明顯相關性。所以我們的結果顯示,CSF中IL-6,IL-8及TNF-α在腦膜炎的過程中扮演者很重要的角色,尤其是細菌性腦膜炎。它們的濃度可以是腦膜發炎程度嚴重性及其預後一個很好的參考指標。 |
英文摘要 | Concentrations of interleukin-6 (IL-6), interleukin-8 (IL-8) and tumor necrosis factor-α (TNF-α) in cerebrospinal (CSF) samples were determined from 11 control and 42 children with central nervous system infections including 11 patients with bacterial meningitis, 20 patients with aseptic meningitis, 11 patients with encephalitis. The CSF IL-6, IL-8 and TNF-α concentrations in patients with bacterial meningitis were significantly higher than those with aseptic meningitis, encephalitis and the control groups. CSF IL-6, IL-8 and TNF-α levels in patients with aseptic meningitis were also significantly higher than those in the control group. There was no significant increase of CSF IL-6, IL-8 and TNF-αconcentrations in patients with encephalitis compared to the control group. CSF IL-6 and TNF-αconcentrations were decreased in patients with bacterial meningitis after treatment. CSF IL-8 levels were significantly decreased in both bacterial and aseptic meningitis groups at recovery period. There were no correlation between CSF IL-6, IL-8 and TNF-αlevels and other parameters including CSF leukocytes, protein, sugar, IgG levels and IgG indexes in patients with bacterial meningitis. These results suggest that IL-6, IL-8 and TNF-a are important mediators in the meningeal inflammatory process in patients with meningitis. The levels of these mediators are good indicators for the extent of the meningeal inflammation |
