英文摘要 | Hemorheoligical and microcirculatory changes may play important roles in the pathogenesis of diabetic vascular complications. To explore tlie interaction of many relevant variables, body mass index, serum blood urea nitrogen, creatinine, triglyceride, cholesterol, hemoglobin Ale, proteinuria and erythrocyte deformability were measured in 80 subjects with non-insulin-dependent diabetes mellitus (NIDDM), 36 subjects with impaired glucose tolerance (IGT) and 34 age-matched normal controls. Among the NIDDM patients, 33 had macrovasculopathies and 27 had diabetic nephropathy. Decreased erythrocyte deformability, as reflected in an increased filtration ratio (FR) was observed in NIDDM subjects with both macrovasculopathy and nephropathy than those without (3.24 ± 0.25 vs. 2.63 ± 0.06, p<0.01). In multivariate analysis, peripheral vascular diseases, cerebral vascular diseases and serum BUN level were the most significant determinants for FR (FR=2.24+0.89(PVD) +0.23 (CVA) + 0.022 (BUN), r=0.69, p<0.001).FR was significantly correlated with fasting serum insulin levels and insulin resistance indices in the 79 subjects, including normal, IGT and NIDDM without macrovasculopathies or nephropathy. In conclusion, impairment of erythrocyte deformability in NIDDM might produce or exacerbate diabetic complications independent ofglycemic control. The association of hyperinsulinemia with various complications might be related to the impairment of erythrocyte deformability. |
