中文摘要 | 本研究之研究目的在:(1)建立護理人員護理能力評估量表,(2)了解進階制度實施前後護理人員的六大能力,(3)比較並評值進階制度實施前、後護理人員六大能力之優缺點,(4)比較護理人員N1, N2, N3, N4進階分級之護理六大能力分數,以作為改進教學及服務品質之參考。研究對象為某醫學中心基層護理人員,採分層隨機抽樣方式,調查進階制度實施前及一年半後護理人員的護理能力。參與前測之護理人員共321位;參與後測之護理人員計222位。前後測問卷信度Cronbachα分別為0.96與0.97,效度採内容及專家效度。用Likert五分法測護理能力。研究結果顯示實施進階制度前護理六大能力,以照護能力平均分數最高(4.04);其次依序為溝通能力(3.95);教學能力(3.83);管理能力(3.69);成長能力(3.60);而研究能力最低(2.85)。進階制度推行前、後的六大能力以t-test比較並無顯著差異,然N1, N2, N3, N4護理人員在六大能力均有顯著差異。六大護理能力中的「照護能力」,以執行醫囑分數最高;確認社會問題分數最低。「研究能力」以查詢護理所需資料分數最高,發表護理研究報告分數最低。未來研究建議將批判性思考能力列入第七項護理能力,再針對護理能力缺失之處加強訓練,以期改善課程設計,進而提昇護理品質及專業能力。 |
英文摘要 | The purpose of this study were to (1) establish nursing competence tool, (2) understand pre and post nursing competence scores in implementing clinical ladder systems, (3) compare and evaluate the nurses' strengths and weaknesses of the six nursing competence, (4) Compare N1, N2, N3 and N4 six nursing competence scores, in order to improve the quality of nursing care. There were 321 and 222 nurses in medical center participated in assessing nursing competence before and after implementing clinical ladder systems. Cronbach's alpha were 0.96 and 0.97 for reliability in pre and post test. Validity was based on content validity and experts' opinion. Five point Likert scale was used. The results showed: (1) The average caring ability were 4.04, higher than communicating (3.95), teaching 93.83), managing (3.69), self-growth (3.60), and research ability (2.85). (2) There were no significant differences in the six nursing competence scores before and after implementing clinical ladder systems. There were significant differences in N1, N2, N3, and N4 nursing competence scores. (4) Among caring ability, the nurses following doctor's order win the highest score, however, identifying social problem get the lowest score. For research ability, collecting data win the higher score, publishing paper get the lower score. The researcher suggest to add critical thinking in the seventh ability and enhance teaching and training in order to elevate the professional ability. |
