英文摘要 | The results of arthroscopic anterior cruciate ligament reconstruction with either the bone-patellar tendon-bone or semitendinosus and gracilis tendons (four strands) were prospectively compared in a consecutive series of 31 patients with chronic injuries. A single surgeon performed operation in an alternating sequence. All cases received surgery with an interval from injury to operation of at least 6 months. The mean follow-up is 18 months (range, 12 to 27). The clinical results for both groups for the Lysholm rating system are shown good to excellent in patellar tendon group was 94% and hamstring group was 84%, and for the IKDC rating system are shown normal to near normal in patellar tendon group was 83% and hamstring group was 85%. Evaluation of symptoms according to the IKDC form revealed hamstring group had less pain but slightly giving way. There were no significant difference between two groups (P>0.05). Anterior tibial displacement was measured with K-T 1000 arthrometer at 30 pounds force, compared to uninvolved knee, in both groups and showed no significant difference (P>0.05). Postoperative isokinetic quadriceps strength testing results comparing the two groups showed no significant difference in peak torque either 60 deg/sec or 240 deg/sec between patellar tendon group and hamstring tendon group. The quadriceps index showed no significant difference in both groups. This result indicate that harvesting either bone patellar tendon bone or semitendinosus with gracilis for autograft ACL reconstruction does not diminish quadriceps strength and functional capacity in our selectively patients who undergo intensive rehabilitation. |
