中文摘要 | 終軸點(terminal hinge axis point)在下顎運動之研究,或臨床上全口贗復 物製作時,常被作為後方解剖參考點之基準。由於下顎骨在發育時之左右對稱性是否勻稱 ,及受測者在作開閉口運動時下顎之偏斜與否,均易影響終軸點定位時左右之對稱性。為 了了解個體終軸點之對稱性,本實驗採高醫牙醫學系20歲學生30名,男27名女3名作為受 測者,以樞紐軸定位器尋找並定位受測者之左右終軸點,並以受測者解剖上之任意樞紐軸 點作為對照點,記錄左右終軸點與任意樞紐軸點之相對位置關係,再比較分析每位受測者 之左右終軸點的對稱性。結果顯示:(1)30 名受測者終軸點與任意樞紐軸點距離之平均 值右側為4.08±1.69mm,左側為3.79±2.23mm。(2)30名受測者左側或右側終軸點與任 意樞紐軸點距離差大於2mm者佔70%。(3)左右側終軸點距離差大於2mm者佔73.1%,大於 4mm者佔39.8%,最大值為13.41 mm,最小值為0.22mm。由本研究結果可知,臨床上以終軸 點作為面弓轉移時之後方參考點時,多數受驗者開閉口向量均有偏斜之現象,其與模型在 咬合器上位置時將有何重要之影響,仍有待更進一步之探討。 # 330 1$aThe terminal hinge axis point (THAP) has been usually used to as posterior reference point in the research of mandibular movement and clinical oral rehabilitation. Since the symmetry of THAP could be affected by the mandibular development and deviation of subject's opening and closing movement, the purpose of this study was to realize the symmetry of THAP among young person. Thirty KMC dental students (27males, 3females) all about 20 y/o were examined. Bilateral THAP were located by using of hinge axis locator for 30 subjects, then symmetrical relationship between bilateral THAP and arbitrary hinge axis point (AHAP) were measured and analyzed using Wilcoxon Signed-Rank test method. The results revealed that (1).the average points difference between THAP and AHAP were 4.08±1.69mm in right side and 3.79±2.23mm in left side (2).among 30 subjects, 70% participants had points difference over 2mm between AHAP and R't or L't THAP (3).for R't and L't THAP difference, the points difference larger than 2mm comprised of 73.1% subjects while 39.8% subjects had difference more than 4mm (maximum: 13. 41mm, minimum:0.22mm). Based on the limited result, majority of the subjects had mandibular opening and closing deviation when using THAP as posterior reference point during face-bow transfer procedure can be concluded. The further studies of the effect of symmetry of THAP on the replication of jaw movement on articulator is needed. |
