中文摘要 | 手術部位感染是一般外科手術最常見的感染之一。本研究目的為了解影響一般外科手術部位感染相關危險因子。採觀察追蹤研究法,自1996年1月至1997年8月以接受一般外科手術步驟之病患為對象。本研究接受外科手術步驟共5,618人次,院內手術部位感染為268人次,平均感染率為4.8%。手術種類中感染率前五位依序為合併肝、胃、腸手術(50%)、胃腸合併手術(21.4%)、腸道手術(17.8%)、肝胃合併手術(13.7%)、肝腸合併手術(12.1%)。影響手術部位感染危險因子具有統計學上意義者包括手術前住院天數、年齡、ASA(American Society of Anesthesiologists)疾病嚴重度、緊急手術、多項手術、手術時間、傷口分類、引流管及手術危險指數。以多變項回歸分析得勝算比前三位為使用引流管(2.3)、緊急手術(2.2)、多項手術(1.88)。共同離出339菌株,以Pseudomonas aeruginosa(12.4%);Staphylococcus aureus(9.7%); Escherichia coli(9.1%)為前三位。此為綜合一般外科所有手術病患資料所得之結果,未來應針對各類不同手術進一步探討其影響手術部位感染之可能因素,了解各種手術間之差異性,以提供臨床更實際有效的參考措施。 |
英文摘要 | Surgical site infection is the most common infection of general surgery. The aim of this study is to prospectively analyze the risk factors of surgical site infections in patients receiving general surgery. From Jan. 1996 to Aug. 1997, a total of 5,618 cases received general surgery, and there were 268 episodes of surgical site infections. The overall infection rate was 4.8%. The highest infection rate occurred in patients receiving combination of hepatic, gastric and intestinal surgeries. The statistically significant risk factors included preoperative hospital stay, age, ASA score, emergency operation, combined operations, the duration of operation, wound classification, the use of drainage tube, and the surgical risk index. The independent risk factors by multiple logistic regression analysis included the use of drainage tube (OR 2.3), emergency operation (OR 2.2), combined operation (OR 1.88). A total of 339 strains of bacteria were isolated. Pseudomonas aeruginosa was the most common isolate (12.4%), followed by Staphylococcus aureus(9.7%) and E. coli(9.1%). In conclusions, our study found that alimentary combined operations had the highest rate of surgical site infection. The independent risk factors included the use of drainage tube, emergency operation, and combined operation. |
