中文摘要 | 對於須要長期使用輪椅的人如下半身癱瘓的脊髓損傷患者而言,壓瘡一直都是相 當困擾的問題;而適當的輪椅坐墊將可以提供預防壓瘡的產生。本臨床評估與分析的目的乃 基於評估由本研究群所研發之可調式坐椅及腎部輪廓量測系統與其研發之電腦輔助設計及製 造的機器去專門製造訂製泡綿坐墊是否可于臨床上改善壓瘡問題。並經由在最大介面壓力與 臨床的評估以了解本發展的效益。在本研究中,受測者分為正常人與脊髓損傷患者。經由介 面壓力系統去評估在不同坐墊下之最大壓力值的變化,並評估對脊髓損傷者取物平衡、轉位 與推行輪椅的影響。實驗結果發現,訂製泡綿坐墊之最大介面壓力值小於未切削之泡綿坐墊 或其它坐墊如脂肪墊,女性受測者壓力分佈較男性平均,而脊髓損傷患者的最人坐壓均值大 於正常人。在臨床評估方面,訂製泡綿坐墊能促進取物平衡與推行輪椅時維持上半身的穩定 。基於本研究中所求之結果,該電腦輔助設計及製造系統的所提供製造訂製泡綿坐墊來減 少壓力,降低引起褥瘡的機會將成為一種有效的臨床及研究的儀器,並促進維持上半身的穩 定。以改善輪椅使用者的身體功能及他們的生活素質。 |
英文摘要 | The long-term wheelchair users, for example the spinal cord injured patients, always have the trouble about the seating pressure sore; and the well-design cushions can offer the wheelchair users to reduce the danger to get the pressure sore. Based on the development of the CAD/CAM system to make custom contoured cushion in our lab, the purpose of this study is to determine whether the custom contoured cushion can reduce the formation of the pressure sore or not. In this issue, we evaluate the clinical effect of the custom contoured cushion by measuring the maximum interface pressure and questioning the spinal cord injured patients participating in the experiment to compare the cushions subjectively. The subjects include the spinal cord injured patients and normal people. The normal people are measured the maximum interface pressure in different types of the cushion by the pressure measurement system. The spinal cord injured patients are evaluated the maximum interface pressure and questioned to compare the cushions subjectively in some clinical items included the seating balance, transfer, and propulsion of the wheelchair. In the results of the experiments, the pressure of the custom contoured cushion is lesser than the uncut foam cushion or other types of the cushion for example the gel cushion. The females have a lesser pressure than the male. The pressures of the spinal cord injured patients are larger than the normal people. In the clinical evaluation, the custom contoured cushion can improve the seating balance and the stability during propelling the wheelchair. According to the achievement of the research, the CAD/CAM system will be an effective instrument for research or clinical use. The custom contoured cushion made by the CAD/CAM system can reduce the interface pressure to prevent the users to get the pressure sore and promote the seating balance and stability, so the custom contoured cushion may improve teh body function and living quality of long-term wheelchair user. |
