中文摘要 | 心室性心律不整(ventricular arrhythmia)常是缺血性心臟疾病如: 心肌梗塞致死的重要因素。近年來研究報告顯示:血管擴張劑可對缺血的 心肌產生保護作用;本計畫的研究對象tetramethylpyrazine (TMP) 已知 可造成大白鼠腦基底動脈、主動脈之血管舒張及抗凝血作用,因此本計畫 進而探討其對大白鼠心肌缺血的保護作用。結果顯示:大白鼠左冠狀動脈 結紮十分鐘前由靜脈注射給予TMP(10 mg/kg)可使心肌因缺血所導致之 心室性心搏過速(ventricular tachycardia;VT)及心室性纖維顫動 (ventricular fibrillation; VF)之發生率明顯降低,並且可延後VF發 生時間(onset)、縮短缺血時VT及VF的發生間期(duration)。另外, 左冠狀動脈結紮後五分鐘,再由靜脈注射給予TMP(10 mg/kg)亦可明顯 降低心律不整(VT及VF)之發生率,並縮短VT及VF的發生間期。缺 血期四十五分鐘結束後給予恢復血液之再循環一小時,而於再循環結束後 以nitroblue tetrazolium染色測量其心肌壞死面積,結果顯示:缺血前 給予TMP (5, 10 mg/kg)均可明顯的降低心肌壞死面積。由以上結果可知: TMP 具顯著的抗心律不整作用,且可明顯降低缺血後之再循環損傷 (reperfusion injury)。 |
英文摘要 | To assess the efficacy of tetramethylpyrazine (TMP), an active component isolated from the rhizomes of Ligustici wallichii, against myocardial ischemia/reperfusion injury, 47 rats underwent 45 min of left coronary artery occlusion and 1 h of reperfusion. Animals were randomly assigned to five groups: control, TMP-2, 5,10, in which TMP (2, 5, 10 mg/kg, i.v. bolus) were administered 10 min prior to ischemia, respectively; post-TMP, in which TMP (10 mg/kg) was administered 5 min after the occlusion of the left coronary artery. Results demonstrated that pretreatment of TMP (10 mg/kg) markedly reduced the incidence of ventricular tachycardia (VT) and ventricular fibrillation (VF) and significantly delayed the onset of VF when compared to the untreated control. Pretreatment of TMP (10 mg/kg) also significantly reduced the total duration of VT and VF. Posttreatment of TMP (post-TMP), was also observed to reduce the incidence of VT and VF. On the other hand, a significant reduction in infarct size, as a percentage of the area at risk, was noted in TMP-5 and TMP-10 groups (control: 66.4±4.8%, TMP-2: 58.2±3.9%, TMP-5:44.8±2.7%,TMP-10:33.4±3.8%, post-TMP:53.9 ±2.8). The results of this study demonstrate that TMP exerts antiarrhythmic and anti-infarct effects during myocardial ischemia-reperfusion. Therefore, TMP would be useful clinically in the treatment and prevention of acute myocardial infarction. |
