中文摘要 | 因為歷史的淵源,在先進國家,醫學和公共衛生的發展是分道進行,醫學教育受到機械論、細菌說的影響,追求生物科學知識的研發與傳遞,忽視社會行為層面的探討,遠離社區大眾的需要;同時因為強調治療疾病,而忽視預防;強調服務個別病患的傳統而較少注意以群眾為基礎的健康問題。再加上專業背景與訓練不同,公共衛生的教師,也很難針對需要的醫學生設計課程。因此,公共衛生教育在醫學教育中一直不受重視。近年來由於反省到醫療科技的發展、醫療費用的高漲並未為國民帶來更好的健康,許多先進國家都採取了不少因應的措施,即醫療照護的改革,例如健保制度的檢討、新興健保制度的開發、健康促進、健康維護與疾病防治的推行,醫學教育者也因而檢討今日的醫學教育能否訓練明日符合社會需要的醫師。公共衛生教育涵蓋了生物統計、流行病學、社會與行為科學、衛生教育、環境與職業衛生、醫院管理、衛生政策等領域,正好可以拓展醫學生的視野,彌補以生物科技為主流的醫學教育之不足,因此廣受醫學教育者的重視。目前許多先進國家都在發展整合性、以學生(未來醫師)為中心的醫學生公共衛生教育,台大醫學院也在1998年進行這樣的嘗試,希望透過更密切的溝通與合作,將公共衛生的理念與作法,自然融入醫學教育中,與醫學院共同培養21世紀符合社會需要的醫師。 |
英文摘要 | Medicine and public health developed separately in developed countries because of different historical backgrounds. Under the influence of mechanic view and germ theory, the medical profession pursues research, development and delivery of biological scientific knowledge rather than socio-behavioral aspects of the patients' health and illness and is away from the need of community. The medical profession also emphasis on disease treatment rather than prevention and individual care rather than the health of the public. Due to the difference in professional training, faculty teaching public health feel it is difficult to design teaching programs for medical students. Therefore, public health has long been an unimportant and unattractive part of medical education. After reviewing the development of technology, cost and unsatisfactory health status of the citizens of their countries, many developed countries have tried a variety of health care reforms, such as a review of health insurance system, development of new insurance plans, campaigns for health promotion and protection, and disease prevention. Medical educators have also begun to review whether the medical education today can prepare physicians to meet the social needs for tomorrow Public health covers a wide range of knowledge including biostatistics, epidemiology, social and behavioral sciences, health education, environmental and occupation health, hospital management, and health policy, Public health education can definitely compensate the insufficiency in contemporary biomedicine-oriented medical education and therefore has attracted the attention of medical educators. Medical schools in many developed countries are now developing integrated, student-oriented public health education. The National Taiwan University College of Medicine is developing it own program since February 1998. The author hope that through closer communication and cooperation this innovative approach will improve concepts and methods of public health in medical education naturally, and help prepare physicians of who can meet the needs of the society. |
