中文摘要 | 背景:遠距鏡影像喉顯微手術乃是在插管全身麻醉下, 以特殊設計的直接喉鏡配 合遠距鏡來暴露喉部病變,並與影像系統結合,使手術不需顯微鏡,而是經由電視螢幕來進 行。 方法:本科自民國 85 年 10 月至 86 年 3 月, 針對 33 名喉部良性病變之病患施以遠距 鏡影像喉顯微手術,並於術前置入傳統喉鏡以為比較。 結果:顯示遠距鏡影像喉顯微手術之視野較佳、景深較廣、器械操作較方便。且臨床觀察發 現具有視覺舒服、可立即顯像與教學效果良好等優點,而其缺點則是必需依賴解析度良好的 電視、無法合併使用二氧化碳雷射、無小孩專用喉鏡以及遠距鏡容易起霧。 結論:此種新的手術方法在目前雖無法取代傳統喉顯微手術,但隨著器械的改進,可望在將 來為喉部疾病之病患提供更精確的治療。 |
英文摘要 | Telescopic Video Microlaryngosurgery (TVM) was performed under general anesthesia by means of endotracheal intubation. The Kantor-Berci laryngoscope with a specially designed telescope was inserted to expose laryngeal lesion. A CCD video camera was then attached to the telescope. Therefour the operation was done under TV monitor without microscope. Thirty-three partients undergoing TVM from October 1996 to March 1997 were studied. Before performing TVM, Nagashima laryngoscope was inserted with Zeiss microscope to compare the ease of laryngoscope insertion, ease of instrument manipulation, visibility and depth of field. The result showed advantages of TVM in instrument manipulation, visibility and depth of field. Besides, TVM was also beneficial in instantaneous documentation and teaching. Inconvenience of TVM included dependence of high-resolution TV screen, inability of laser use, no pediatric laryngoscope and easy fog. |
