中文摘要 | 由於複合樹脂在聚合收縮與高熱膨脹係數的缺點下,易使窩洞邊緣與材料之間出 現脫離,進而造成患者對冷熱敏感,齲齒再度發生或牙髓病變等後遺症。此外,牙釉質為人 體中最硬但最脆弱的組織,常於窩洞修形後因結構的缺損而造成咬頭斷裂。為了改善窩緣牙 釉質 - 樹脂交界面的黏著強度,減少邊緣微滲漏現象, 進而達到強化牙齒的作用,臨床上 乃應用酸蝕黏著之技術。近年來牙本質黏著劑之研究開發使得市面上之酸蝕劑種類繁多,主 要是以較弱的酸同時對牙釉質和牙本質進行所謂 " 全酸蝕 "(total etch) 的處理。本實驗 的目的即在比較各種酸蝕劑之酸蝕形態與作用速率,進而明瞭其適用範圍。吾人乃應用具有 低真空試片室之掃描式電子顯微鏡來觀察經酸蝕作用後牛牙釉質結構之連續性變化。結果顯 示:(1) 不同的酸蝕劑有不同的酸蝕形態與作用速率。(2)10% citric acid 作用 1 分鐘後 僅能移除塗抹層,而不能達到酸蝕的變化。(3)2.5% nitric acid, 10% maleic acid 和 10% phosphoric acid 作用 1 分鐘後均能達到明顯的酸蝕形態。(4)10% phosphoric acid 作用 3 分鐘後會造成牙釉質稜柱斷裂的過度酸蝕變化。 (5) 這四種酸蝕劑若以廠商所推薦的 15 秒時間來作用,似乎並不能產生明顯及穩定之牙釉質酸蝕。 |
英文摘要 | The acid-etch technique is based on establishing a mechanical bond between the composite bonding agent and enamel. Traditional treatment of enamel by 37% phosphoric acid offers an attractive approach for increasing adhesion, reducing microleakage at the cavosurface interface, and further, providing cuspal reinforcement. Recently, several new adhesive systems rely on the simultaneous etching of dentin as well as enamel with weaker acids. This study was designed to analyze the morphological and dynamic changes of enamel etching with different etchants. Sequential observations of the uncoated enamel under a scanning electron microscope equipped with a low vacuum specimen chamber were performed. The results of this investigation demonstrated that: (1) The topography of bovine enamel etched with different etchants is not alike. (2)Etching enamel with 10% citric acid for 1 minute only removes the smear layer. It doesn't etch the enamel. (3) Using 2.5% nitric acid, 10% maleic acid and 10% phosphoric acid as enamel etchants etches the enamel optimally in one minute. However, the order of the etchning was:2.5% nitric acid>10% phosphoric acid>10% maleic acid. (4) Using 10% phosphoric acid may cause fracture of enamel rods in 3 minutes, revealing an overetched appearance.(5) Accordingly, etching enamel with weaker acids for 15 seconds as recommended by manufacturers may be insufficient to produce an optimal bond of composite to enamel. |
