中文摘要 | 呼吸訓練與痰液清除是慢性阻塞性肺病 (chronic obstructive pulmonary disease, COPD) 患者肺復健中首階段之復健訓練項目,亦是近二十餘年來醫學界極為關切與努力探討的問題,但在國內尚無人對此做過研究。本研究目的在探討呼吸訓練與痰液清除護理方案對COPD患者呼吸道清除與其他症狀的成效。本研究採準實驗法,立意選取某醫學中心二十位COPD住院患者,實驗組與對照組各十人。實驗組施行呼吸訓練與痰液清除護理方案,對照組則接受一般的護理。研究者評估所有研究對象的COPD進展,包括呼吸道狀況、活動之運動耐受度、與呼吸道清除效果。以SASPC進行描述性之統計比較,與分析性之統計檢定,本研究結論為呼吸訓練與痰液清除護理方案能縮短慢性阻塞性肺病患者的住院日數,但是呼吸道狀況、最高呼氣流量、活動之運動耐受度、及呼吸道清除失效定義特徵的改變程度,在統計上均無顯著性差異。 |
英文摘要 | Breathing training and pulmonary hygiene are the basic items in the first section of the pulmonary rehabilitation program for chronic obstructive pulmonary disease (COPD) patients. They are reported to benefit COPD patients. This study was designed to ascertain the value of breathing training and pulmonary hygiene nursing protocol for relief of ineffective airway clearance and other symptoms in COPD patients. A Quasiexperiment with single-blind design was used to evaluate the effectiveness of this program. Twenty incident cases at a medical center were purposively assigned to either experimental or control group. Patients in experimental group were received a breathing training and pulmonary hygiene nursing protocol, They were taught to practice twice a day during hospitalization period. In control group, patients received common conventional nursing interventions. All study patients were evaluated the prognosis of COPD such as airway condition, activity tolerance, and defining characteristics of ineffective airway clearance. The study showed that breathing training and pulmonary hygiene nursing protocol effectively shortened lengths of stay in hospital. The data also revealed some improvements in study variables such as the sputum, peak flow meter, activity tolerance, and the scores of defining characteristics of ineffective airway clearance, however, they were not statistically significant at alpha level of 0.5. |
