中文摘要 | 鎮動力鏈運動在許多方面已證實比開放動力鏈運動更具功能性與安全性,尤其 是針對前十字韌帶受傷的病人,但卻極少研究探討此種運動的訓練效果。本研究之目的為探 討屬於閉鎖動力鏈運動型 的下肢推蹬訓練對肌力和功能性表現的影響。10名健康受試者進 行為期 4週,每週 3次的下肢推蹬訓練。男12名健康受試者則不接受任何肌力訓練。所有受 試者在訓練後均受(1)股四頭肌╱大腿後 的等速向心肌力,(2)下肢推蹬肌力,(3) 單腳跳遠與(4)單腳計時跳躍等測試。在訓練組方面,100°/sec與 240°╱sec的股四頭 肌峰值力矩、所有速度的下肢推蹬峰值力矩、單腳跳與單腳計時跳躍的能力在訓練後都有顯 著的改善(p<.05),其他測試結果則無顯著差異。在控制組方面,僅180°╱sec與240° ╱sec的肌四頭肌峰值力矩所有速度的下肢推蹬峰值力矩、單腳跳遠與單腳計時跳躍的能力 在訓練後都有顯著的改善(p<.05),其他測試結果無顯著差異。在控制組方面,僅180°/sec 與240°/sec的股四頭肌峰值力矩 有顯著變化(p<.05)。訓練後的下肢推蹬肌力與功能性表 現有明顯的進步,顯示本篇的訓練方式有其效果,並符合訓練特殊性的觀念。而訓練效果無 法完全反應於開放動力鏈的測試方式,可能是肌肉收縮型態不同所致。既然多數的下肢功能 動作屬於閉鎖動力鏈的運動型態,復健與評估的方法也應利用閉鎖動力鏈運動的方式,才能 將訓練效果充分轉移至功能性活動中,並在合適的測試中反應出來。 |
英文摘要 | Closed kientic chain (CKC)exercise have been promoted as more func- tional and more safe than open kinetic chain (OKC)exercises, particularly for injuries involving the anterior cruciate ligament.Limited research demonstrated the training effect of CKC exercise on muscle strength and functional perform- ance.The purpose of this stydy was to determine the effect of a leg press train- ing on isokinetic peak torque and functional performance of lower extremity.Ten subjects participated in a 4-week training period,and 12 subjects served as con- trols.Pre-and posttests included:(1)isokinetic strength test of quadriceps/ hamstring,(2)leg press test,(3)single-leg hop test for distance, and(4) single-leg hop for time.For the training group,a paired t-test comparing pretest and posttestvalues revealed significant improvements(p<.05)in the isokinetic quardriceps peak torque at 180°/sce and 240°/sec,leg press peak torque at all three speeds, aingle-leg hop for distance, and single-leg hop for time and no significant chang in other variables. For the control group,the significant difference(p<.05)was found only in the isokinetic quadriceps peak torque at 180°/sec and 240°/sec.We conclude that this legpress exercise protocal improved lower extremity performance as measured by CKC activities, and the results supported the concept of training specificity.The OKC testing mode was not sensitive enough to detect actual improvement in lower extremity performance related to CKC training, and the possible cause may be the difference of muscle activation pattern between CKC and OKC exercise.Since the lower extremities usually function in a CKC mode, rehabilitation and assessment should be perform- ed in a CKC.Therefore, any training effect would have a beneficial carryover to functional activities, and be detected by those appropriate tests. |
