中文摘要 | 從1984年五月至1987年四月,共有五位痙攣性斜頸的病人,在本院接受外科治療,年齡在20至53歲之間,其中四位接受兩側脊椎內C1,C2及C3前神經根切除及兩側第11對腦神經的脊椎副神經切除術。另一位接受脊椎內右側Cl,C2及C3前神經根切除及兩側脊椎副神經切除術。有兩例有顯著的血管壓迫到副神經的現象,但是經顯微血管去壓術後,沒有效果;在第二次手術接受前神經根切除術後得到改善。一例左上肢無力,可能是因為臂神經叢被臉頰壓迫所致,手術後獲得改善。整體而言,五例接受手術者,術後追蹤檢查,病情都有明顯的改善。其中有兩例病人輔以藥物治療,效果更佳。 |
英文摘要 | Five patients with spasmodic torticollis, ages ranging from 20 to 53 years, were treated at our hospital from May 1984 to Apr. 1987. Four received bilateral anterior rhizotomies of C1, C2 and C3 and resection of both spinal accessory nerves; one received bilateral spinal accesory nerve resection and anterior rhizotomy of right C1, C2 and C3. In two cases marked distortion of the spinal accessory nerve roots caused by vascular compression was disclosed, but vascular decompression offered no symptomatic relief. Significant improvement was obtained following rhizotomies in a second operation. In one case left upper limb weakness, which might have been due to brachial plexus damage by a severely deviated cheek, was relieved after operation. The overall results of bilateral C 1-2-3 ventral rhizotomies with resection of the spinal accessory nerves in our patients were satisfactory both subjectively and objectively, although in two cases medication was still needed to obtain good control of neck posture. |
