中文摘要 | 近年來,電腦斷層掃描影像雖然使神經外科醫師在診斷上更為方便,但在手術過 程中二維的影像仍不足以做精確定位。本研究發展了國內第一套達實用性的腦部手術用規劃 與導引系統,此系統包含影像處理與重建,定位裝置及方位校準等三大單元。影像重建是以 行進方塊法將二維掃描影像重建出立體影像,並求出手術器械端點所在的三個正交剖面與任 意剖面影像。在標記的位置與手術器械的方位量測方面,採用磁場式定位裝置。方位校準則 是以標記為媒介,再由基因法則找出影像座標與定位裝置座標間的方位轉換關係。手術前醫 師於影像中所規劃的手術路徑可經由此轉換關係以定位裝置在病患頭上精確的定出,而手術 進行中,手術器械的方位也轉換成相對於影像座標的方位,並於影像中即時顯示出手術器械 所在的組織剖面影像。由臨床實驗中證實本系統可協助醫師於手術前規劃安全的手術過程並 於手術中精確的定位,其定位精度可達 1.77 mm 左右,對提升手術品質有相當的助益。 |
英文摘要 | Recently, CT images provide impressive assistance for neurosurgeons to diagnose diseases. However, the information available from two-dimensional scanned images is not enough for precise positioning of surgical instruments. This paper presents the first set of planning and navigation system for neurosurgery in Taiwan. This system is composed of three units: image processing and reconstruction, tracking system, and registration between the image frame and tracker frame. Image reconstruction unit generates the 3D model from 2D images by using Marching Cube algorithms. The three orthogonal cross-section views of the brain, which pass through the position of the surgical instruments. The registration between the tracker frame and the image frame is determined on the base of the coordinate transformation of the physical markers and their images by applying genetic algorithms. Through the transformation, preplanned surgical paths in the image frame can be mapped to and indicated on the patient. During intraoperation, the position of surgical instruments can be mapped to and shown in the image system, too. From clinic experiments, we verify that the navigation system can assist surgeons to preplan safe surgical paths and to precisely position surgical instrument during intraoperation. Its precision is about 1.7mm. It can certainly improve surgical quality. |
