英文摘要 | Total Hip Replacement (THR) usually causes operation failure by loosening. The wear debris grinding from artificial femoral head and acetabular cup leads the interaction of tissues and cells around this place to inducing the osteolysis is the most commonly suggestion. No matter what kind of material it used, the important study is the wear debris of these materials how to effect the cells in culture. Hydroxyapatite (HAP), had been tested many times, is a well compatibility bioceramic. It was grouped to four sizes (A. 0.3 ∼ 5 μ m, B. 37 ∼ 63 μ m, C. 177 ∼ 250 μ m, D. 420 ∼ 841 μ m), and added to osteoblasts culture medium of 3 cm-diameter well with 10 �� cells as the same concentration (1 mg/ml). This experiment has five groups including A ∼ D groups and E group (control group) which is no material, and each group has 10 wells. They were harvested after 1 hour, 3 hours, 1 day, 3 days and 7 days, and checked the concentration of prostaglandin E �� (PGE �� ), tranforming growth factor beta 1 (TGF β 1) and the activity of alkaline phosphatase (ALP) with enzyme linked immunoassay (ELISA). Then take the pictures from inversed microscopy and measure the nuclear length, cell area, and cell number with image analysis system to see the grouth of cells. In A group, the concentration of TGF β 1 is lowest, the value of PGE �� is highest and the cell number is very less compared with each group of B ∼ E groups and they have significant difference. In B,C,D groups, the concentration of TGF β 1 is similar to E group and sometimes it will exceed the value of E group, the value of PGE �� is higher than E group but smaller than A group and the cell number is less than E group and more than A group. But ther is no significant difference among B ∼ D groups. The value of ALP and nuclear length have no significant difference (ANOVA P>0.05). The cell area has two kinds of effects on A ∼ D groups. The one is A group, and the other one is B ∼ D groups compared with E group. These results shows that there is a mechanism of cell damage in vitro which depends on a direct interaction between cells and small particles (0.5 ∼ 3 μ m) and is largely independent of the chemical nature of the larger particles (37 ∼ 841 μ m.) The small particles will decrease the concentration of TGF β 1, increase the concentration of PGE ��, decrease the cell number, increase the number of pycnotic cells seriously, and then cause many harmful problems between cells and tissues. So we should pay attention to the small wear debris caused by artificial femoral head and acetabular cup to prevent the loosening of THR. |
