英文摘要 | Revision of the failed hip arthroplasty with massive bone loss on the femoral side is a difficult task, especially when it is associated with a deep infection. We report 13 patients having revision total hip arthroplasty using structural or strut cortical allografts for femoral deficiencies due to aseptic or septic failures. There were seven male and six female patients with mean age of 54.8 years. Eight patients had aseptic and five, had septic failures of previous hip arthroplasty. Eleven hips had a circumferential and two had a noncircumferential femoral deficiency. A proximal femoral allograft (PFA) was used in nine, proximal tibial allograft (PTA) in two and cortical strut allografts (CSA) in two hips. At a mean follow-up period of 3 8 months, 10 patients (77%) had an excellent or good result. Two were fair and one was poor. The mean Harris hip score improved from preoperative 27.3 to postoperative 72.5. Eighty-five percent of the grafts have united to host bone. Complications included two dislocations, two nonunions, two breakage of trochanter wire and one infection. No hips in five revisions for septic failure developed recurrence of infection. Our results support the use of structural or cortical strut allografts in revision of sepfic or aseptic failure after total hip arthroplasty. However,it demands an experienced surgeon and good surgical equipments. |
