中文摘要 | 細菌性眼感染疾病在臨床上佔相當重要的一環。雖然抗生素的廣泛使用,一般的 病例已能控制,但治療極為棘手的角膜潰瘍及眼內炎等重度感染,則需完全依賴更嚴謹的抗 生素療法。而目前市面上的抗生素點眼製劑,在種類及濃度上均有限。所以,我們傳統上採 用抗生素針劑,自製成適當濃度局部點眼液。但此類點眼液,因由注射劑配製而成,其安定 性較差。一般建議需冷藏保存,且以短期間經常更換為原則。因而造成了使用上的不方便。 本計劃的目的,乃希望對於自製抗生素點眼液,研究出一理想的保存方式以及有效使用期限 。如此一來,不僅能減低經常換藥水的不方便,另一方面,亦能一次備製多瓶點眼液,而有 效地保存起來,不致造成抗生素針劑之浪費。臨床常用之自製抗生素眼藥水包括 1% Amikin 、 10% Pipril 及 5% Vancomycin, 以及其 5 ∼ 10 倍高濃度保存液包括 10% Amikin、 50% Pipril、 25% Vancomycin, 以 5% 葡萄糖溶液稀釋後, 分別儲放於 4 ℃、-18 ℃及 -80 ℃, 以 Pseudomonas aeruginosa 測試 Amikin、 以 Staphylococcus aureus 測試 Pipril 及 Vancomycin, 分別於抗生素製成後、及 3、7、14 及 28 天之儲存後, 測試其 MIC 值。 大部分之 MIC 值之變化均在 7 天內產生,於 7 天至 28 天則呈現較穩定之情形 。 比較第 0 天和第 28 天之變化,只有儲存在 -80 ℃之 10% Pipril 及 25% Vancomycin 其 MIC 值為有意義之變化, 本實驗測試之自製抗生素眼藥水可於 4 ℃及 -18 ℃儲存 28 天之久。 高濃度之儲存並不提高抗生素之效力,於 Pipril 濃度越高反而效力越低。於 28 天之實驗期,不同溫度儲存並不影響抗生素之效力。 |
英文摘要 | Self-prepared fortified antibiotic eye drops are essential for the severe ocular infection. The relation of potency decay and storage conditions including temperature, concentration and duration were studied by the changes of MIC. 1% and 10% amikin, 10% and 50% pipril, and 5% and 25% vancomycin were diluted from the prarenteral antibiotics with the 5% glucose and storage at 4,-18 and -80 ℃ for 3,7,14 and 28 days. MICs of amikin against Pseudomonas aeruginosa, pipril and vancomycin against Staphylococcus aureus were determined by the agar diffusion method. Fluctuations of the MIC were noted during the observation period. Most of the significant changes of MIC were found during the first 7 days. When the potencies between time of zero and 28 days are compared, only 10% pipril and 25% vancomycin stored at -80 ℃ had significant change. Our conclusion is that all of the fortified antibiotic eye drops in this study can be stored in the house refrigerator or freezer for up to 28 days. High concentration may show a negative result of the preservation. Temperature does not influence the preservative effect within four weeks' observation. |
