中文摘要 | 利用干擾素(Interferon α-2a)在實驗中能夠阻止血管內皮細胞遷移和增 殖之特性,有可能治療老年性黃斑部變性,遏止新生血管之生長。其效率和可行 性如何是本研究所要探討的。本計劃收集20例老年性黃斑部變性網膜下新生血管 病患,隨機分成治療組和對照組。治療組給予干擾素每平方公尺體表面積300萬 單位,每週三次肌肉注射,連續八週。對照組則不接受任何治療。所有病患定期 以螢光血管攝影觀測新生血管大小,並記錄出血和滲出液範圍以及矯正視力的變 化情形,兩組作比較分析。 結果發現治療組新生血管範圍增加1例,稍為減少1例,不變8例,對照組增加2 例,減少1例,不變7倒。治療組出血增加1例,減少2例,不變7例,對照組增加 者1例,滅少2例,不變7例。治療組滲出液增加者1倒,減少1例,不變8例;對照組 增加4例,不變6例。在視力方面治療組退步3例,不變7例;對照組退步5例,不變 5例。在治療的全身副作用上,7例有全身倦怠、頭痛、肌肉酸痛現象。 從所得到的結果顯示干擾素無法封閉新生血管,亦無法明顯地使出血和滲出液減 少,在視力方面也沒有進步。我們結論以現行之干擾素α一2a投與方法對老年性 黃斑部變性新生血管之治療並無顯著效果。 |
英文摘要 | We performed a prospective, randomized clinicalstudy to evaluate the effect of interferon a -2a onsubfoveal choroidal neovascularization secondary toage-related rnacular degeneration (ARMD). Twentypatients who received no previous treatment of anyform before this study were enrolled. Ten patientswere randomly assigned to the treatment whereasanother ten patients of matched age were assigned toobservation alone as controls. Interferon a -2a wasadministered by intramuscular injection of 3.0 millionunits per square meter of body surface area, threetimes a week for eight weeks. Fluorescein angi-ography, fundus photography and best correctedvisual acuity tests were done before treatment andevery two to four weeks. All patients were followed up for a minimum of 4 months. The results showed that during the course oftreatment and at the end of follow up, there were nodifferences in visual acuity changes, neovascularprogression rates, or the extents of hemorrhage andexudation between two groups of patients. Mostpatients had side effects which included fatigue,myalgia, headache, fever, nausea and anorexia. In this study, interferon a -2a failed to improvevisual acuities or regress subfoveal neovascularmembranes in ARMD. The treatment was alsoassociated with marked systemic side effects. It isconcluded that interferon a -2a with currentlyavailable regimen is not useful in the treatment ofchoroidal neovascularization from ARMD. |
