英文摘要 | Noninvasive, continuous cardiac output monitors using the thoracic electrical bioimpedance technique were undertaken in nine normotensive term pregnancies to investigate the hemodynamic effects of uterine contractions in the early first stage of labor. All parturients were placed in the left lateral decubitus position without any analgesia. Peak flow index (PFI) and end diastolic index (EDI) increased significantly during the peak of a uterine contraction. Ejection fraction was not affected by firm contraction. Maternal heart rate increased (3%) slightly, however it was not significant Statistically. A 15% increase in stroke index (46±8.28 to 53±7.58 ml.beat/m□) was associated with an overall 18% rise in the maternal cardiac index 38±0.87 l.min/m□ to 45±0.85 l.min/m□) (p<0.005). Hemodynamic change in this study suggest that uterine contractions increase maternal stroke volume through cardiac preload augmentation from the blood expressed by each uterine contraction. |
